Heir of Perdition is a Mythic Unique helm in Diablo IV. Blizzard’s 3.1 patch notes record a major reduction to its damage bonus, from 80% to 15%, and a change to Mother’s Favor. For acquisition, Blizzard documents general Mythic Unique routes; a current third-party item database specifically reports that Heir of Perdition can drop from uber bosses.
What is Heir of Perdition?
Heir of Perdition is a Mythic Unique helm, so equipping it uses the helm slot that could otherwise go to another helm. Blizzard developers have described that competition as a balance concern: Lead Systems Designer Art Peshkov said in a June 23, 2026 developer discussion that some items “dominate the meta,” naming Heir of Perdition. Senior Game Designer Charles Dunn said, “Heir of Perdition took out the option for other helms.”
Those comments explain why the item’s strength and the opportunity cost of the helm slot matter, but they do not establish a best-in-slot ranking for a particular build. A useful comparison with another helm depends on how well each fits the build, its item power and affixes, its unique effect, and what the build gives up by using that slot.
What does Heir of Perdition do, and was it nerfed?
Yes. Blizzard’s [3.1 patch notes](URL) document the damage bonus reduction: “Damage reduced from 80% to 15%.” The notes also say: “Mother’s Favor now instead steals 15% Critical Strike Chance from other players and always retains its damage bonus.” These are the changes recorded in those 3.1 notes; they do not establish whether a later update changed the live item again.

How do you get Heir of Perdition?
Blizzard’s Season of Death Awakening announcement describes two general Mythic Unique acquisition routes. It says players on Seasonal and Eternal Realms have a very rare chance to find a Mythic Unique whenever an Ancestral Unique would drop, and it describes crafting Mythic Uniques with slot-based targeting. These are general system rules, not confirmation of Heir of Perdition’s item-specific loot table.
