Batman was infected in Batman: Arkham City, not during the events of Batman: Arkham Knight. Joker injected his own Titan-contaminated blood into Batman while he was incapacitated, using the infection to force Batman to help find a cure. Batman’s “Story So Far” narration, reproduced by Arkham Wiki, also says Joker contaminated Gotham’s blood supply.
How Joker infected Batman
In the account of Arkham City’s story, Joker incapacitated Batman and transferred his blood into him with a syringe. Batman explains the motive in the game’s recap: “To ensure my cooperation he infected my blood with his own, leaving me no choice but to help him.” The blood was contaminated by Joker’s exposure to the Titan formula, which was making him seriously ill. The quoted narration is reproduced by Arkham Wiki, a secondary reference.

Joker’s plan extended beyond Batman: the same recap says he contaminated Gotham’s blood supply too. The infection was therefore both a way to pressure Batman and a threat to other people in Gotham.
Why it comes up in Arkham Knight
Arkham Knight follows the earlier infection and reveals its continuing consequences. Batman does not get newly infected by Joker during the events of Arkham Knight; the source of the blood-related plot is the syringe incident in Arkham City.

Was Batman cured in Arkham City?
The official Batman: Arkham Knight PC manual recaps that Joker was dying from exposure to Titan, that Batman and Mr. Freeze developed a cure, and that Joker accidentally destroyed it before succumbing to his illness. That summary establishes the cure’s role in Joker’s story, but it does not explain whether the cure removed every later consequence of Joker’s blood in Batman. The game’s recap supports the fact of the infection and its coercive purpose; it does not justify a more specific medical explanation.
