For the final Hiruko fight in Nioh 3, in the main mission A Sinister End, watch his attack patterns, preserve Ki, and punish him after an attack ends. The encounter also includes a Yokai Realm phase, where ranged Ninjutsu and available Summoning Seals can help. These tips cover the final story fight, not the earlier Hiruko encounter associated with Ill Omen.
Which Hiruko fight is this?
Hiruko appears in more than one encounter according to search-result listings, which distinguish an earlier fight in Ill Omen from the final story encounter in A Sinister End. PowerPyx identifies the latter as the final Hiruko boss fight. The strategy below is for A Sinister End; do not assume every detail applies to the earlier encounter.

How to approach the final Hiruko fight
Lock on and learn the attack rhythm
Lock-on keeps Hiruko centered and makes attacking and dodging easier while showing his Life and Ki. At the start, focus on reading his movements instead of rushing in. The official Nioh 3 Official Web Manual recommends defending and observing unfamiliar enemies, then counterattacking after an attack ends and leaves them open.
Keep enough Ki to defend or escape
Attacks and guarding both consume Ki. If it reaches zero, you cannot attack or guard, leaving you exposed if Hiruko catches you. Avoid long attack strings when your Ki is low; create distance and briefly stop attacking and guarding to recover. In Samurai Style, use Ki Pulse after attacks to regain some of the Ki you spent.

Switch styles to suit the situation

The manual advises choosing the style that feels easiest, then learning to use both as situations demand. You do not need to commit to one style for the whole fight.
Punish the end of his attacks
Once Hiruko finishes an attack, use the opening to strike, then be ready to defend or dodge again. PowerPyx describes his melee attacks as slow and relatively easy to dodge, and recommends attacking from behind when you can reach a safe angle. Treat that as a strategy suggestion, not a guaranteed opening: if you are unsure whether an attack has finished, wait rather than overcommitting.

Use ranged options in the Yokai Realm phase
PowerPyx recommends using Ninjutsu and available Summoning Seals during the Yokai Realm phase while working to deplete Hiruko’s Ki. It also advises keeping some distance from his elemental attacks. Adjust your response to the attack you see rather than assuming distance makes every move safe.
Weapons and optional tools
PowerPyx recommends Dual Ninja Swords in Ninja Style for this fight, but its general boss guidance says any weapon can defeat any boss and encourages using a weapon that feels natural. Dual Ninja Swords are an option, not a requirement. Prioritize a weapon you can use without exhausting your Ki, and take short punish opportunities rather than forcing a long combo.
If you have Life Leech available, PowerPyx suggests using it. The guide does not establish that it is necessary, or that a particular build or weapon is required to win.
