FFmpeg concat failures do not have one universal fix: the cause may be a malformed list file, incompatible streams, incorrect duration metadata, or a mismatch between the command and the files. Start by saving the full command and error output, inspect every clip, then use the concat demuxer only when the inputs are compatible; use the concat filter when they need normalization or re-encoding.
Identify where FFmpeg fails
The title alone does not reveal the root cause. A precise fix depends on the complete FFmpeg command, its stderr output, the FFmpeg version, and the properties of the source videos. Save those details before changing anything.
Locate the stage where the failure occurs:
- List parsing: FFmpeg cannot interpret the concat text file or its header.
- Input opening: A listed path is wrong, inaccessible, or rejected by safe-path handling.
- Stream matching: The clips have different streams, codecs, or time bases.
- Packet writing or finalizing: The output or timestamp behavior may be involved; the full error and media details are needed to narrow this down.
Do not assume that every concat error is a looping problem. Repeating a clip through a concat list still requires a valid list and compatible media.
Inspect the clips before choosing a method
Use ffprobe or FFmpeg’s input stream summaries to compare every file. Check the number and order of streams, video and audio codecs, time bases, dimensions, frame rates, audio layout, and reported duration. The exact inspection command depends on your shell and installed FFmpeg version, so use a command suited to your setup rather than relying on a one-size-fits-all probe.

The concat demuxer expects matching streams, codecs, and time bases across files. FFmpeg’s documentation states: “All files must have the same streams (same codecs, same time base, etc.).” (FFmpeg concat demuxer documentation.) If a story clip has a different stream layout or timing base, stream-copy concatenation may fail or produce unwanted results.
Choose concat demuxer or concat filter

Try stream copy for compatible clips
Create a text file with one file entry per clip, in the order you want them to play, then run this illustrative starting point:
ffmpeg -f concat -i list.txt -c copy output.mp4
This is not a guaranteed fix: the demuxer works only when the files fit its constraints. Do not use it to conceal differences that require normalization.

Use the concat filter when you need an encode
When the clips need to be made consistent, use FFmpeg’s concat filter and encode the result with settings chosen for the actual source media and intended output. FFmpeg’s FAQ says of the concat filter: “This operation is recommended if you need to re-encode.” (FFmpeg FAQ.) Re-encoding costs processing time and can reduce quality if settings are poor; it also gives you a route to a uniform output.
Fix concat list syntax and file paths
If FFmpeg is meant to recognize the list automatically, its first line must be exactly ffconcat version 1.0. Do not add a space or a byte-order mark before it. Use one file line for each input and verify that every path points to an existing file.
Spaces and special characters in paths can cause parsing trouble if they are not escaped or quoted correctly. The demuxer’s safe option defaults to 1 and rejects paths it considers unsafe. Setting -safe 0 permits any file name, but use it only for a list you trust and when those paths are actually required. See the concat demuxer documentation for the option and list-file rules.

Check duration and timestamps
The demuxer schedules each next file using the previous file’s duration. If a file reports an incorrect duration, later segments can shift and the result may contain artifacts. Different audio and video stream lengths can also create gaps because FFmpeg adjusts timestamps across the sequence.
A duration directive can override a file’s reported duration, but add one only when you know the correct duration. It is not a general-purpose timestamp repair. If the actual duration is uncertain, investigate the media and the error output before overriding it.

Loop one story clip or assemble several
Decide whether you need to repeat one source a fixed number of times or fill a target runtime; those are different tasks. If you repeat the same file by listing it multiple times, each entry is still subject to the concat list syntax, compatibility, and duration requirements described above. The right looping command depends on the desired number of repeats or runtime and on the source file; do not treat a loop command as a fix for an unrelated concat error.

Prepare the assembled video for YouTube
YouTube’s current upload guidance recommends MP4, H.264 video, AAC-LC or Opus audio, and keeping the source frame rate. Its standard computer aspect ratio is 16:9, while the player also adapts to vertical and square uploads. These are recommendations, not universal acceptance requirements. See YouTube’s recommended upload encoding settings.
For SDR 1080p, YouTube recommends these bitrates:

These figures apply to SDR 1080p recommendations; do not apply them indiscriminately to other resolutions or HDR. Keep the frame rate aligned with the source rather than changing it merely to match a bitrate row.
Troubleshoot common symptoms
- The list is not recognized: Check that the first line is exactly
ffconcat version 1.0, without leading spaces or a byte-order mark. - FFmpeg cannot open a listed clip: Confirm the path, filename, permissions, and list-file quoting or escaping. If a trusted list uses paths rejected by safe handling, consider
-safe 0only when necessary. - Stream-copy concat fails on some clips: Compare stream count and order, codecs, and time bases. If they do not match, choose an encode-based filter workflow instead.
- Later segments shift or show artifacts: Check reported durations and timestamp behavior. Use an explicit duration only when its correct value is known.
- Audio or video has a gap: Compare audio and video stream lengths in each input; unequal lengths can affect the adjusted sequence timestamps.
If the cause remains unclear, share the full command, unabridged stderr, FFmpeg version, and stream summaries for every input. Without those details, naming a single root cause would be guesswork.
