In Survival mode, the Institute’s outbound fast-travel destination is limited to the C.I.T. ruins. If the Institute has turned hostile while you are inside, take the central elevator to the relay room, press the red emergency relay button beside the main console, and enter the room to teleport out.
Leave by fast travel after Institutionalized
Completing Institutionalized unlocks fast travel to and from the Institute. Open the Pip-Boy map, choose a discovered Commonwealth destination, and confirm fast travel. The quest reference describes this as the normal way to get out once the quest is complete. Fallout Wiki: Fallout 4 quests

What changes in Survival mode?
In Survival mode, outbound fast travel from the Institute is limited to the C.I.T. ruins, according to the Institute location reference. Select the C.I.T. ruins on the Pip-Boy map rather than expecting to travel directly to another discovered location. Fallout Wiki: The Institute
Escape if the Institute is hostile
- Go to the central elevator and ride it up to the relay room.
- Press the bright red emergency relay button beside the main console.
- Step into the relay room. A countdown begins, then the relay teleports you out.
If Father is alive, he may speak over the intercom during the countdown. If he has been killed, an Institute scientist speaks instead. Fallout Wiki: Banished from the Institute

What makes the Institute hostile?
The emergency-relay route applies when you have become an enemy of the Institute. The Banished from the Institute walkthrough identifies quest and faction choices that can trigger hostility after Institutionalized, including killing Father or another named Institute character; responding negatively to Father at the end of The Battle of Bunker Hill; choosing the Brotherhood route in Spoils of War by boarding the Prydwen vertibird for Mass Fusion; or supporting the Minutemen during Pinned, subject to that quest’s prerequisites. Simply walking out of the Institute is not, by itself, listed as a cause of hostility. Fallout Wiki: Banished from the Institute
If you were banished while outside, the walkthrough says you can return to the Institute location and use the relay route to complete the escape objective, provided you can still access the location.
