Use RadAway from your inventory to lower your accumulated radiation. For a complete removal, visit a doctor. Check your Pip-Boy radiation meter or status to see how much radiation you have.
Use RadAway to reduce your rads
RadAway is the portable, consumable option. The game manual describes it as an item that flushes rads from your system; its Xbox 360 instructions say to press A to use it, but controls differ on other platforms. See the Fallout: New Vegas manual for the platform-specific context.

RadAway removes more radiation at higher Medicine skill. The Fallout Wiki lists these values:

Those figures are from the Fallout Wiki’s item reference; they are not a promise that every version or modded game behaves identically. In Hardcore mode, RadAway’s effect is gradual rather than immediate. Check your radiation status again after it has had time to take effect.

Rad-X is different: it increases resistance to radiation exposure. It is not the primary item for removing rads.
See a doctor for full removal
A doctor can remove all accumulated radiation, according to the Fallout radiation reference. This is the option to choose when you want a complete reset rather than reducing rads with a consumable. The reference does not establish exact locations or availability for every doctor.

Some additional removal methods are tied to particular downloadable content, so they are only relevant if you own and can reach that content:
- Old World Blues: the Sink Auto-Doc is listed as an option.
- Lonesome Road: the Irradiated Beauty perk and fiery purgative are listed as options.
- Cave fungus: the radiation reference also lists it, but does not establish the exact requirements or availability here.
For these alternatives, the available reference does not establish exact locations, stock sources, or all prerequisites. Consult the relevant in-game content or item description before relying on one.
