What an Uncut Skill Gem does
An Uncut Skill Gem is a currency item. Its description reads, “Creates a Skill Gem or Level an existing gem to level #,” and instructs you to right-click to engrave a Skill Gem. Use it to select a skill gem to create, or to level an existing skill gem up to the level shown on the uncut gem. Path of Exile 2 item reference

How the gem level is determined
The level printed on the Uncut Skill Gem sets the level of the gem it can create or the maximum level it can bring an existing gem to. The item reference lists drop-level breakpoints: for example, drop level 1 corresponds to a level 1 gem, drop level 4 to level 2, and drop level 7 to level 3. At drop level 62, the table includes skill and spirit gem levels 14–15; at drop level 80, it includes levels 17–20. These are possible levels listed for those drop levels, not a guarantee that a particular monster will drop a specific item level. Consult the breakpoint table rather than assuming character level alone determines the result.
Where to get one
Quest reward: Reaching Clearfell
The quest-item variant is a level 1 Uncut Skill Gem. The item reference says it is awarded for completing Reaching Clearfell and obtained in Clearfell Encampment during Act 1. It works like a regular level 1 Uncut Skill Gem. Quest item reference
Other drops
Uncut Skill Gems also come from drops. Their available gem levels depend on the drop-level breakpoints in the item reference, rather than a simple character-level rule. The same reference records a version 0.3.0 change in which uncut gems accumulate equity toward higher-level drops in higher-tier maps; that is a documented change for that version, not confirmation of how every later patch behaves. Uncut Skill Gems are for skill gems. Uncut Support Gems are for support gems, while Uncut Spirit Gems are for persistent-buff skills. These are separate item types, so make sure the uncut gem matches the kind of gem you want to engrave. Path of Exile 2 item reference

Drop behavior and quest rewards can change between patches, so if an exact current drop or reward is important, verify it in the live game or official patch notes.
