If your PS4 is stuck on a system-software update screen and the controller will not let you press X, connect the DUALSHOCK 4 directly to the console with a known-good USB data/sync cable, then press PS. Safe Mode and update-recovery screens are designed around a wired controller connection, not normal Bluetooth pairing. If that fails, reset the controller, try both USB ports, and only then move to update recovery. Do not choose Initialize PS4 (Reinstall System Software) until you accept that it erases the console.
Before changing anything
- Write down the exact message and code, such as SU-30746-0, SU-41350-3, SU-42118-6 or another code. Code-specific instructions are maintained in Sony’s PS4 error-code guide.
- Disconnect external drives, USB hubs and other accessories. Leave only the controller connected.
- If the console is actively installing an update, do not pull the power cable. Interrupting an installation can damage system software.
- Do not select an initialization or reinstallation option while you are still diagnosing the controller.
Fastest way to restore controller input
- Turn the PS4 off completely.
- Connect the controller directly to the PS4 with the original cable or a confirmed data-capable micro-USB cable. Many cables charge but carry no data, so the light bar can come on while button input is ignored.
- Try the other PS4 USB port. Do not use a hub, extension or adapter.
- Press the small reset button in the hole beside the controller’s L2 button for about 3–5 seconds. Sony documents this procedure in its DUALSHOCK 4 support guide.
- Reconnect the cable and press PS. A blue light normally indicates that the controller has paired.
A reset cannot repair a broken cable, damaged controller socket, failed USB port or console hardware fault. If the controller has no light at all, test another cable, controller and power source before assuming the update caused the problem.
Enter Safe Mode correctly
- Hold the PS4 power button for about three seconds to shut it down.
- Press and hold the power button again. Release it after the second beep; Sony says the second beep is approximately seven seconds after the first press.
- Connect the controller by USB and press PS. Simply plugging in the cable is not enough.
Follow Sony’s current sequence and menu descriptions in the PlayStation Safe Mode guide. Wireless pairing is not the first solution on this screen because Safe Mode starts only basic console functions.
Use the least-destructive Safe Mode choices first
- Select Restart System once. A temporary failed download may clear.
- If the error returns, enter Safe Mode again and choose Update System Software.
- Use Restore Default Settings only if you need to reset settings; it does not fix a bad cable or USB port, and your settings will be lost.
Rebuild Database scans the storage drive and creates a new content database. It is appropriate for missing or duplicated icons, database corruption or startup problems after controller input has been restored. It does not repair a charge-only cable, defective controller, unreadable update file or failed USB hardware.

Retry the update from the dashboard
If the controller works and you can reach the dashboard, delete the failed download: open Notifications, highlight the update, press OPTIONS, and select Delete. Then go to Settings > System Software Update and try again. A wired internet connection is preferable when available. Sony recommends using USB recovery when normal downloads repeatedly fail; see the PS4 system-software instructions.

- Format the USB drive as FAT32 or exFAT.
- Create a folder named
PS4. - Inside it, create
UPDATE. - Save the standard update file as
PS4UPDATE.PUP. - Insert the drive directly into the PS4, start Safe Mode, and select Update System Software > Update from USB Storage Device > OK.
Folder names and the filename must use the exact spelling and uppercase format. If the PS4 says the file is not recognized, check the format, capitalization, filename, download integrity, partition layout and USB drive itself. The ordinary update file is different from the larger reinstallation package.

These are broad descriptions, not proof of one failed component. The same frozen screen can result from the controller, cable, USB port, update file, storage drive or console.

Understand the data risk of every Safe Mode option

Sony warns that some Safe Mode choices cause data loss and recommends backing up to USB storage or cloud storage before using destructive options. Cloud storage requires the applicable PlayStation Plus availability; a USB backup is preferable when the dashboard still works.
Last resort: reinstall system software
Use Initialize PS4 (Reinstall System Software) only after the controller works, the official update file and USB setup have been checked, and you accept losing saves, captures, installed games and settings. Download the reinstallation package from Sony’s PS4 system-software page, place it in the required PS4/UPDATE structure, start Safe Mode and choose option 7. Reinstallation can correct software corruption, but it cannot fix a failed USB port, defective controller or deteriorating hard drive.

When the evidence points to hardware
- One controller fails but another known-good controller works: suspect the first controller or its USB socket.
- Several controllers fail with several data cables: suspect the PS4 ports or console hardware.
- The controller works on another device, but neither PS4 port recognizes it: the console is more likely at fault.
- USB storage is also not recognized, the console repeatedly loops into Safe Mode, power behavior is abnormal, or different official files and drives all fail: stop repeating reinstalls.
Use Sony’s PS4 support and PlayStation Repairs diagnostic path for persistent console or controller connectivity faults.
