This reference covers the symptom list published by PC Gamer on 14 January 2026 for the game released on 12 January 2026. The labels and readings below are reported game mechanics, not real-world medical guidance. Tools and entries may unlock as the campaign progresses, and the in-game journal should take precedence if a later patch changes a result.
The game’s checkpoint loop asks you to screen survivors and decide whether they proceed, are quarantined, or receive further laboratory screening, as described in the Steam store listing. A symptom’s classification matters: the guide groups findings as safe, quarantine-worthy, or infection/liquidation indicators. An unfamiliar respiratory sound is a reason to send someone for laboratory testing, not to guess.

The table follows the symptom and tool categories in PC Gamer’s symptom guide. The game’s unusual organism and symptom names are in-game labels, not clinical diagnoses. Where the source guide leaves a category empty or a placement uncertain, check the current in-game journal instead of assuming a result belongs in that category.
How to inspect a survivor without missing signs
- Make a full visual pass. Use the scanner and inspect the survivor from multiple angles. Check the back, feet, and areas partly hidden by clothing; a front-facing glance can miss visible signs.
- Take both vital readings. Use the thermopulsometer and compare temperature and BPM separately with the game’s categories. Upgrades can add color indicators or sound cues, but the readings are the values to match against the symptom list.
- Test reflexes. Strike an arm or leg with the reflex hammer and watch which limb moves. A crossed reflex means the opposite limb responds.
- Listen at the lungs. Place the stethoscope over the black spots on the lungs, checking both the front and back. Ordinary wheezing can be a safe finding; wheezing on the inhale with a silent exhale, sometimes accompanied by clicking, is listed for quarantine; lung growling indicates infection. If a sound is unfamiliar or absent from the journal, route the survivor to laboratory testing.
- Check internal signs and hidden items. Cycle through all four X-ray views to inspect organs and reveal hidden contraband. A hunched survivor holding their stomach may deserve closer attention, but that posture alone does not establish a diagnosis. Search baggage separately.
- Use the Matioscope and UV when available. Pan across each Matioscope layer and adjust focus to inspect for anomalies. Once UV is unlocked, activate it at the luggage table to reveal glowing signs on survivors and belongings.
How to distinguish look-alike findings
Red bruise versus necrosis
The guide describes a red bruise as deeper purple, while necrosis appears cool black with small blood-red sections. Because both can be visually confusing, scan carefully rather than classifying by a quick glance.

Different breathing sounds
Do not treat every wheeze as infected breath. In the guide’s descriptions, ordinary wheezing can indicate sickness while remaining safe; wheezing on inhale with a silent exhale (sometimes with clicking) is a quarantine cue; and lung growling signals infection. Send an unrecognized sound for laboratory testing.
Green skin and skin tone
The green-skin sign can appear yellow depending on a survivor’s skin tone. Consider the other visible signs and tool results instead of relying on hue alone.

What the readings and categories mean in play
The listed bands put values below 37°C and below 120 BPM in the safe group, 37–41°C and 120–180 BPM in quarantine, and 41–45°C and 180–200 BPM in the infection/liquidation group. These are game thresholds reported by the guide, not medical cutoffs. The table does not establish classifications beyond those ranges.
A safe result is not the same as an infection finding, and a quarantine result is not the same as an infection/liquidation finding. When a tool provides a clue but not a listed symptom, or when the respiratory result is unclear, use the game’s laboratory-screening route rather than promoting an uncertain finding to infection.

This symptom inventory reflects PC Gamer’s guide dated 14 January 2026. A free DLC prequel, Miami Zero, was announced for 15 October 2026 in a Steam Community announcement. Because the symptom list is a secondary guide rather than an official complete database, later campaign content or patches may differ; verify changes against the in-game journal.
