What does taking the girl’s hand do?
The choice near the beginning of Shin Megami Tensei V: Vengeance selects one of two story routes: taking the hand chooses the Canon of Vengeance, while declining chooses the Canon of Creation. It is a route choice, not a choice about the protagonist’s gender.
The game gives you a chance to reconsider before you confirm. Once confirmed, the route is set for that playthrough; to experience the other route, start a new save. Dot Esports explains the confirmation prompt and save requirement.
How the two routes differ

Which route should you choose?
Choose Vengeance for a first or only playthrough
If you have not played the original, or expect to play only once, Vengeance is the better fit if you want the new material and a more character-focused story. Nintendo Everything’s guide also characterizes its ending requirements as less complicated.
Choose Creation to revisit the original story
If you are returning to the game specifically for its original narrative, choose Creation. It is also the more suitable route if you prefer an ending shaped by an endgame choice rather than choices made across the story.

Plan ahead if you want completion or Godborn Mode
Nintendo Everything reports that full completion requires four Creation playthroughs and two Vengeance playthroughs. Its guide also says completing Creation first with a particular ending condition can unlock Godborn Mode for the first Vengeance playthrough. Because these are guide-reported completion details, check the guide’s conditions before committing to that route order: the specific ending condition is a spoiler.

You can change your mind at the confirmation prompt, but not after confirming the route for that save. If you want the other story later, you will need to begin a new save rather than switch the existing playthrough.
