According to coverage of the clip, the staff respected his wish to remain anonymous—so they saw his face, but kept his identity private. That explanation comes from entertainment-news reports about the footage, not an official statement from the filmmakers.
What happens in the deleted hospital scene?
GamesRadar reports that the footage fills in a gap after Peter is rushed in for emergency surgery: in the operating theater, medical staff remove his Spider-Man mask. SuperHeroHype describes Peter as wounded, gasping and frightened as the medics treat him. The theatrical cut skips this moment and next shows Peter recovering in his hospital bed.
The clip was reported as part of a featurette on the digital release. It is deleted footage, not a scene in the theatrical version.
How does the scene explain Peter’s hospital anonymity?
The staff can see Peter’s face when they remove his mask, but SuperHeroHype reports that they honor his request to keep his identity secret. That is the answer the scene offers to how Peter managed to keep his identity private in the hospital: the people treating him knew who he was and did not reveal it. This is the outlets’ interpretation of the released clip; the sources do not cite a separate official explanation from Sony, Marvel or the filmmakers.
