The sigil marking the spell eventually faded, but Wizards of the Coast does not describe that as an escape.
How Emrakul ended up in Innistrad’s moon
Nahiri drew Emrakul to Innistrad as part of her revenge against Sorin. The Eldrazi titan’s presence warped the plane and its inhabitants; Innistrad’s people came to know the crisis as the Travails. The Gatewatch ultimately chose imprisonment over trying to destroy her.
In the climax of the official Eldritch Moon story, a silver beam draws Emrakul up toward the moon, where Tamiyo’s spell seals her. Wizards’ design retrospective identifies the card Imprisoned in the Moon as depicting Tamiyo’s decisive act (Wizards of the Coast’s “Eldritch Perfect, Part 1”). The official fiction, “The Promised End” by Ken Troop, shows the spell’s climax. The setting guide describes Innistrad’s moon as made entirely of Moonsilver.
Is Emrakul dead, or still trapped?
She is still trapped, not dead. Wizards’ Planeswalker’s Guide to Innistrad: Midnight Hunt says the great sigil on the moon eventually faded from view, but Emrakul still resides inside it, “slumbering quietly.” The faded mark is not presented as evidence that she escaped.

Wizards’ later set commentary likewise distinguishes Emrakul from the other known Eldrazi titans: Ulamog and Kozilek are dead, while Emrakul remains imprisoned in Innistrad’s moon. The status is also reflected in commentary on Midnight Hunt, which explains the absence of Eldrazi creatures in that story context (“Odds & Ends – Innistrad: Crimson Vow, Part 1”; “Odds & Ends – Innistrad: Midnight Hunt, Part 2”).
What happened to Innistrad after the Travails?
The madness and physical corruption associated with the Travails dissipated after Emrakul was sealed away. That did not make Innistrad entirely peaceful: later, sunlight began to wane, nights grew longer, and frost descended. In the Midnight Hunt setting guide, astronomancers and the Dawnhart Coven suspect that these changes may be connected to Emrakul’s presence in the moon. The account presents that connection as a suspicion, not a confirmed cause.

Why aren’t there Eldrazi on Innistrad now?
Emrakul is a titan, and Eldrazi creatures are extensions of their titans. With her imprisoned and slumbering, her presence does not mean that active Eldrazi are roaming Innistrad. Wizards’ Midnight Hunt commentary uses her continued imprisonment to explain the lack of Eldrazi creatures in that set’s story context; it does not establish that she has been destroyed or that the prison is permanent.
