If you choose SPARE during the Genocide Route’s Sans fight, Sans kills the protagonist and the game returns to the last SAVE. Resetting afterward starts a new run; it does not trigger the lasting consequences associated with completing Genocide and accepting the fallen human’s offer.
What happens when you spare Sans?
SPARE is not a mercy ending or a way to leave the Genocide Route peacefully. The attempt ends in a game over, and the game’s normal game-over flow returns you to the most recent SAVE. See the Undertale Wiki’s Sans page for the fight details.

Reset is a separate action from continuing after a game over. It abandons the current attempt and clears most active save information; it does not complete the route or accept any post-ending offer. The Undertale Wiki’s Reset page describes the reset mechanic.
Does resetting after the fight cause a permanent change?
The cited route and reset references describe no special persistent consequence for sparing Sans and then resetting. The lasting change people associate with Genocide happens later, after completing that route and responding to the fallen human—not when Sans kills you or when you reset after the game over.

Even after reaching the Genocide Route’s point of no return—the final required Hotland/CORE kill before Mettaton NEO—you can still reset before answering the fallen human at the ending. The route’s point of no return means the run has passed a route-progress threshold; it does not mean you have already accepted the post-ending offer.

Yes. A fresh attempt can lead to True Pacifist, but the route has its own requirements: first complete a Neutral run, kill no monsters, befriend Papyrus, Undyne, and Alphys, and explore the True Lab. The Undertale Wiki’s True Pacifist Route page outlines those conditions.
