What regular parasite specimens do
Each regular specimen you consume unlocks one Illithid Power in a skill tree separate from ordinary character leveling. Larian Studios described the tree as containing 25 abilities in its 2023 Community Update; that figure is tied to that update, not a fresh count for every later game version.
Larian also says companions can consume specimens and may perceive you differently depending on that choice. This is an optional gameplay system, distinct from the later partial-illithid transformation.
How the two parasite choices differ

What happens if you use the Astral-Touched Tadpole
The Astral-Touched Tadpole is a distinct, later decision—not simply another ordinary specimen. The cited mechanics reference says that accepting its powers makes the character partial-illithid and changes their appearance. It describes communing with or eating the tadpole as ways to receive its powers; if you eat it, another party member cannot commune with it.

The appearance change is described as irreversible through the Magic Mirror. The claim that the transformation does not consequentially change story choices comes from a community-maintained mechanics reference, not an official developer guarantee. That reference also notes a small set of companion comments.

The official developer description establishes the powers as an optional progression system, separate from leveling. It does not establish that using ordinary specimens inevitably causes the Astral-Touched Tadpole transformation. Keep the choices distinct: consuming regular specimens unlocks powers; the special tadpole is the decision associated with partial-illithid status and the appearance change.
