The five-defeat requirement is a cumulative boss-defeat unlock for The Polaroid in Rebirth. It unlocks the item; the cited progression reference does not say that the fifth defeat itself immediately triggers a new ending.
How this fits into nearby unlocks
The Polaroid is one of several separate progression milestones. Do not confuse its condition with the unlocks tied to Satan or Mom’s Heart.
