Deathroot is a key item, not a crafting ingredient or consumable. Its practical purpose is to give it to Gurranq, Beast Clergyman, at Bestial Sanctum in eastern Caelid. Each hand-in advances his reward sequence, and the ninth Deathroot grants an Ancient Dragon Smithing Stone. The Beast Eye obtained early in the sequence also warns you when another Deathroot is nearby.
The item reference lists nine Deathroots in total. The locations below follow that list; dungeon names and boss pairings should still be checked against your in-game map or an accessible current reference, because the detailed list has received less direct verification than the item and NPC descriptions.
What Deathroot does
Deathroot occupies the key-item category in Elden Ring. You cannot consume it for a stat effect, use it in crafting, sell it for a normal material payoff, or equip it. Take it to Gurranq at Bestial Sanctum and select the dialogue option to give him Deathroot.
Gurranq is in the Bestial Sanctum in the distant east of the map. His exchange quest is the item’s documented use: nine turn-ins provide incantations, an Ash of War, weapons, a seal, a warning item, and an upgrade stone. After the first offering, Gurranq gives the Clawmark Seal and the Beast Eye. The Beast Eye displays a warning when you are near another Deathroot, making it useful while exploring even though Deathroot itself has no combat effect.

The following is the nine-entry list attributed to the Eldenpedia Deathroot reference. It is organized by acquisition rather than an assured progression route; Elden Ring does not require you to collect them in this order.

Five entries in the list come from Tibia Mariner or other field-boss rewards, while four come from dungeon chests. If a chest is not present, defeat the named dungeon boss first and search the boss arena’s reward area rather than expecting the root to appear as a dropped field item.

The first reward is especially useful for a character using bestial incantations: the Clawmark Seal is the catalyst tied to that spell family, and the Beast Eye gives an exploration cue for the remaining roots.

After the fourth Deathroot, Gurranq attacks the player in Bestial Sanctum. This does not mean the quest has failed. Reduce his health by 30 percent; he then becomes subdued and can be spoken to again. Continue the exchange sequence normally afterward.
The same NPC reference connects Gurranq with Maliketh, the Black Blade. That connection is a story reveal rather than a separate Deathroot function. After all nine roots have been given, the reference says Gurranq leaves the sanctum.

What Deathroot means in the lore
The in-game item description says:
“A source that gives rise to Those Who Live in Death.”
“The Beast Clergyman, found at Bestial Sanctum in the distant east, collects and devours these roots.”
“On the night of the dire plot the stolen Rune of Death enabled the first Death of a demigod. Later, the Rune of Death spread across the Lands Between through the underground roots of the Greattree, sprouting in the form of Deathroot.”
