In Dungeons & Dragons 4th Edition, a creature was bloodied when it had half its maximum hit points or fewer. That threshold was more than a description: specific powers and effects could target bloodied creatures or work better against them. The term returned in the 2024 rules, but Bloodied has no effect by itself unless another rule refers to it.
How Bloodied worked in D&D 4th Edition
A creature’s bloodied value was generally half its maximum hit points. Once its current hit points reached that value—or fell below it—the creature was bloodied. The 4th Edition rules explain that “Certain powers and effects work only against a bloodied target or work better against such a target” (Wizards of the Coast, Dungeon Master’s Kit, 4th Edition).
This made half hit points a tactical breakpoint. A power might become available only against a bloodied target, or an effect might become stronger against one. Bloodied did not automatically impose a universal penalty or change every creature’s behavior; the specific power or rule determined what happened.
The bloodied value and character death
In 4e, the bloodied value also had a separate role in the player-character death rules: it was relevant to how far into negative hit points a character could go before dying. That rule did not mean that becoming bloodied itself caused a character to lose actions or suffer a general penalty.

What Bloodied means in the 2024 rules
The official 2024 Basic Rules define the threshold as half your Hit Points or fewer, then clarify that Bloodied “has no game effect on its own but which might trigger other game effects” (D&D Beyond Basic Rules, 2024, “Playing the Game”). In other words, the marker tells you that a creature is at or below half its hit points; a separate rule must say what, if anything, follows from that.

The shared half-hit-point threshold does not make the editions’ mechanics identical: 4e used the value in more explicit combat and death-rule contexts, while the 2024 definition treats it as a marker for other rules to invoke.

What “bloodied” meant in 2014 fifth edition
In 2014 fifth-edition guidance, a DM could describe a monster at half its hit points as bloodied, giving players a broad sense of its condition without disclosing an exact hit-point total. That descriptive use should not be mistaken for 4e’s explicit bloodied value and keyed powers, or for the 2024 glossary definition. The 2014 use described a monster; it did not establish Bloodied as a general status with automatic rules consequences.

No. In both 4e and the 2024 definition, the threshold is half hit points or fewer. A creature qualifies at the halfway point and remains bloodied below it. The label also does not require the DM to reveal the creature’s exact hit-point total.
