Start the healthy backup when the questionable player’s availability or likely workload is uncertain and you cannot use a later replacement if news turns bad. Start the questionable player when credible late information supports playing, his expected fantasy value is meaningfully higher, and you still have a usable fallback before your lineup locks. “Questionable” means uncertain—not a 50-50 forecast, a guarantee of playing, or a prediction of workload.
What “Questionable” tells you—and what it doesn’t
The NFL defines Questionable as uncertain whether a player will participate in the game. The designation is not a probability estimate, and it does not say how much a player will play if active. The NFL’s policy also distinguishes practice participation from game status; treat them as separate pieces of information, not interchangeable projections. See the NFL’s injury-report policy revision and its 2016 Injury Report Policy. The current NFL injury report shows practice status and game status separately and updates during the week.
The NFL policy says, “If there is any question concerning a player’s availability for the game, he should be listed as ‘Questionable.’” That wording comes from the policy article published August 2, 2016; it describes the designation, not an individual player’s odds of being active.
Compare likely fantasy value, not status labels
The decision is between two players’ expected contributions under your league’s scoring rules. Estimate the questionable player’s value across plausible outcomes—active with a normal role, active but limited, or inactive—and compare that with the healthy backup’s expected role. Being active does not by itself make a player a strong start, and a healthy backup is not automatically better.

This is a decision framework, not an NFL-published projection. No universal point margin makes risk worthwhile: scoring format, role, projections, matchup situation, and the availability of a fallback all affect the choice.
Check whether you can wait for more information
Kickoff order can preserve—or remove—your options. If the questionable player’s game starts later and your backup is still unlocked, you may be able to wait for a status update. If the backup plays first, starting him may lock that lineup spot before you can use the later player. Check the exact rules for your fantasy service and league rather than assuming every platform works the same way.

For NFL.com Fantasy Football, the 2025 rules say users can change a player’s active or reserve status until that player’s NFL team is scheduled to start; NFL.com’s beginner guide also describes players locking at kickoff. Those rules apply to NFL.com, not automatically to other services. Consult the NFL.com Fantasy Football Official Rules 2025 and beginner’s guide for its rules, then verify the settings in your own league.
A practical start-or-sit process
- Check the official report. Look at the current NFL injury report and separate the practice listing from the game-status designation. Do not translate Questionable into “50-50.”
- Assess the possible roles. Consider whether the questionable player could be inactive, active but limited, or active with his usual opportunity. Use credible team updates, but do not treat a fantasy designation as medical clearance.
- Compare expected points. Evaluate both players using your league’s scoring settings and realistic role expectations. If the questionable player’s likely value is only marginally better, the healthy player may be the safer choice when you cannot react later.
- Check kickoff and lock times. Confirm when each player’s team starts and when your platform locks that player or lineup spot. Identify a specific eligible fallback before deciding to wait.
- Choose for your matchup. If you are projected to protect a lead, favor reliability when the expected values are close. If you need upside, accepting more uncertainty may make sense when the questionable player’s potential contribution is substantially stronger.
- Recheck before the applicable lock. Update the lineup if status changes and a replacement remains available. The deadline depends on your platform and league settings.
When each choice makes sense
Start the healthy backup when uncertainty cannot be covered
- The questionable player’s availability or workload is still unclear.
- You will not have an eligible replacement after the relevant lineup lock.
- The backup’s expected contribution is competitive with the questionable player’s risk-adjusted value.
Consider the questionable player when the upside is actionable
- Credible late information supports that he will play and his expected role remains useful.
- His expected fantasy value is meaningfully higher than the backup’s.
- You retain a viable fallback until the status is clearer or the lineup locks.
Neither choice can be made from the Questionable label alone. The useful question is whether the questionable player’s expected edge is worth the risk you cannot hedge in your particular league.
