All three Elder Guardians are inside an ocean monument: one is in the top room and one is in each of its two wings. Check all three areas; ordinary Guardians elsewhere in the monument are different mobs and do not mean another Elder Guardian is hiding nearby.
Where to look inside the monument
- Top room: One Elder Guardian is in the monument’s top room.
- First wing: One is in one wing.
- Second wing: The third is in the other wing.
The tutorial labels the wings left and right, but those labels depend on which way you are facing as you move through the structure. The dependable approach is to search both wings rather than use a compass direction. Minecraft Wiki: Ocean Monument
How to find an ocean monument
Ocean monuments generate in deep ocean biomes and are rare. The official Minecraft overview advises looking in deep water rather than shallow areas. Minecraft: Ocean Monuments
Explore or use an ocean-explorer map
You can search deep ocean biomes for the structure, or use an ocean-explorer map to guide you to one. Cartographer villagers sell these maps for emeralds, according to the Minecraft Wiki. Minecraft Wiki: Ocean Explorer Map

Why you may still see Guardians
Regular Guardians are distinct from Elder Guardians. The monument’s three Elder Guardians occupy the top room and its two wings; seeing regular Guardians after checking those locations is not evidence of a fourth Elder Guardian. Minecraft Wiki: Guardian
