The guide covers PlayStation consoles generally, so where Bethesda’s patch notes name PS4 specifically, this article says so. The official material contains seven distinct actions, not nine, so this is a seven-fix guide. None of these steps is a guaranteed cure, and Bethesda does not claim that one cause explains every crash.
The seven official steps, in order
- Make sure the game is fully updated. Bethesda’s guide asks players to confirm the game is fully updated before anything else. Several PlayStation crash fixes shipped in later builds, so an outdated copy is the first thing to rule out.
- Close and restart the game. Press the PS button to return to the dashboard, close Fallout 4 from the Options menu, then launch it again.
- Power cycle the console. Select Turn Off PlayStation rather than Rest Mode. Wait until the console light is off, unplug the console’s power for 10 seconds, reconnect it, and restart. Bethesda describes this as fully turning the console off, unplugging it, waiting 10 seconds, and reconnecting it.
- Test with Creations disabled. Turn Creations off from the game’s Creations menu and play to see whether the crash continues. If you need to narrow things further, delete an individual Creation from the Library. If the crashes stop only while Creations are off, Creation content is involved, but the official article does not say which Creation is responsible.
- Reinstall the game. Remove the game, then reinstall it from the Library or from the game disc. Save files are handled separately; see the reinstall section below.
- Try launching offline. Disconnect the console from the internet and launch the game. Bethesda lists this as a fallback after the earlier steps.
- Contact PlayStation Support. Bethesda directs persistent problems to PlayStation Support for further troubleshooting. The guide does not offer a diagnostic tool or repair beyond that.
Why the update comes first
Bethesda’s patch notes show that PlayStation crash and stability work arrived in several separate updates, some of them tied to Creations and DLC. The table lists each dated entry as Bethesda describes it, along with the platform scope the notes state.

Patch history helps identify an outdated installation or a known content-specific symptom. It cannot show what happened on your particular console or in your particular save.
Match the crash to its likely cause
The patch history suggests a few patterns. Use them to decide which step to prioritize, but treat them as leads rather than diagnoses.

- Crash during or after a Creation download, or when returning to the main menu mid-download: confirm the game is fully updated, since the May 27, 2026 notes address this exact sequence.
- Missing DLC after installation: check the DLC installation and apply updates. Bethesda ties this symptom to a PS4 Anniversary Edition fix, but missing DLC is not the cause of every crash.
- Crashes after enabling Creations before DLC finished installing: Bethesda advises enabling Creations only after DLC is completely installed.
- Crash mentioning a streaming-audio error: Bethesda’s fix is not labelled PS4-specific. Update the game, then follow the steps in order rather than assuming the fix applies to your console.
Reinstalling without losing saves
Reinstalling is step 5, and it carries the most reader anxiety. Bethesda’s guide is direct on the point: “This process will not delete your game save files, but only the game itself.” Uninstalling removes the game program, not your saves.
Before you uninstall, back up your saves. Bethesda’s guide mentions backing up saves online through Application Saved Data settings. It does not give a menu path, so look for Application Saved Data settings in your console’s system settings and confirm your saves are stored online before removing the game.

After removal, reinstall from the Library or from the game disc. The disc is only one reinstall route. It does not fix a software crash by itself, and the Anniversary Edition DLC fix shows that the edition you own matters, so check that your disc or listing matches the version you play.
Offline launch and when to contact support
Launching offline is a diagnostic step, not a permanent fix. It changes network availability for that session. If the game runs cleanly offline while crashes persist online, that points toward network-linked content or downloads, but Bethesda does not state this relationship in its guide. Reconnect the console to the internet once you have finished testing.

If the earlier steps fail, PlayStation Support is the next stop. Bethesda’s guide does not describe what support will check, so bring the details you have gathered: which steps you tried, the approximate date the crashes began, whether Creations or DLC were involved, and whether the crash happens offline.

What the official guidance does not establish
- Bethesda’s support and patch material includes no crash-rate, success-rate, or prevalence figures, so no step can be expected to fix a given crash.
- No single cause is named for all PlayStation crashes. The notes describe several separate issues across different updates.
- Patch notes do not diagnose an individual console, account, or save file.
