Find the first four Skandhas in Chapters 1–4, then use them to unlock the optional Giant Shigandang fight in Chapter 6. After the fight, collect the fifth Skandha and give all five to Xu Dog to receive the Five Skandhas Pill, which slightly increases health, mana, and stamina.
This spoiler-inclusive route follows the chapter order. Three Skandhas are environmental pickups, one is a side-boss reward, and the last is behind an optional endgame boss.

Chapter 1: Skandha of Form
Forest of Wolves — Front Hills
From the Front Hills shrine, cross the wooden bridge into the forest. Head toward the Buddha-face rock in the left-front corner of the area, before the Bullguard encounter, and interact with it to collect the Skandha of Form.

Chapter 2: Skandha of Feeling
Fright Cliff — Rockrest Flat
This Skandha requires a detour before the boss encounter: collect all six Buddha’s Eyeballs in Chapter 2. Then go to the Stone Vanguard arena and interact with the rock there to deliver the eyeballs and summon Shigandang. Defeat Shigandang to obtain the Skandha of Feeling.
Chapter 3: Skandha of Thought
Valley of Ecstasy — Mindfulness Cliff
From the Mindfulness Cliff shrine, move onto the wooden boards and drop down to the right. Follow the path to the stone head and collect the Skandha of Thought.

Chapter 4: Skandha of Choice
Webbed Hollow — The Purifying Spring
Follow the path from the shrine to the pond on the left. Go through the pond to find a Buddha-head sculpture holding the Skandha of Choice.
Chapter 6: Skandha of Consciousness
Mount Huaguo — Foothills, Verdant Path
Once you have the first four Skandhas, travel beyond the main lake from Verdant Path using cloud flight. Giant Shigandang is in an optional area near the end of the upper area, in a narrow canyon. Defeat the boss, then climb its arm to collect the Skandha of Consciousness.

Turn in all five Skandhas to Xu Dog
Having the complete set is not enough by itself; take the five items to Xu Dog and choose the hand-in option. Xu Dog can be found at the Chapter 2 Cellar shrine or in Zodiac Village if you have unlocked it. The reward is the Five Skandhas Pill. Guides describe it as slightly increasing health, mana, and stamina; exact increases are not stated in the cited route guide.
