In Toriel’s first battle in Undertale, choose Spare repeatedly and do not attack her. The encounter can end with Toriel spared; that is separate from completing the game’s True Pacifist route.
How to spare Toriel in the first battle

On your turn, select Spare rather than attacking Toriel.

Keep choosing Spare as the encounter continues. A community speedrun route guide reports that Toriel eventually avoids attacking and then stops; continue selecting Spare until the encounter ends. See the route guide.

Does sparing Toriel get the True Pacifist ending?
No. Sparing her in this battle is not enough on its own. A community-maintained route summary says a Neutral ending must be completed first and that the player must not kill monsters to fulfill the True Pacifist requirements. Read the route summary.
Can Frisk stay with Toriel at the end?
Yes. In the True Pacifist ending, the player can choose whether Frisk stays with Toriel or leaves. That choice changes the post-credits scene, according to the route summary.

What “save Toriel” means
If you mean keeping Toriel alive in her first fight, repeatedly choosing Spare is the relevant action. If you mean reaching the True Pacifist ending and seeing the option to stay with her, that depends on the broader route requirements as well. These are related outcomes, but sparing her in the encounter does not itself complete that route.
