Yes. During the Run! quest, Henry can save Theresa from the Cumans attacking her near the horses as he escapes Skalitz. Whistle to distract them, then keep moving toward the nearby horse and Talmberg. Rescuing her earns the Cavalier achievement or trophy; missing the moment does not prevent her later appearance or Courtship quest.
How to save Theresa during Run!

After the Cuman attack on Skalitz, follow the escape route downhill toward the buildings and horses. Theresa calls for help nearby.

Use your game’s whistle control to draw the attackers’ attention. The guides list X on PC, Y on Xbox One, and Triangle on PS4 in the Cavalier guide; the Run! guide lists Y on Xbox and Triangle on PS4. If those labels do not match your platform or setup, use the whistle control shown in your game.

What happens if you save her?
Successfully rescuing Theresa earns the Cavalier achievement or trophy. The encounter is easy to miss because the escape objective encourages you to keep moving.

Can you still pursue Theresa if you miss the rescue?
Yes. Missing the rescue does not remove Theresa from the later story: she still appears to rescue Henry, and her Courtship quest remains available, according to PowerPyx’s Theresa romance guide. The guide supports her later appearance and Courtship quest; it does not establish other romance consequences.

Prima Games says an autosave is created at the start of Run!. If you missed the rescue, load that save to retry the opportunity. Save behavior can depend on the game and circumstances, so check that the relevant autosave is available before relying on it.
