The blast can kill you and ignite nearby blocks, but death is not guaranteed; the outcome depends on the circumstances. If survival matters, do not use a bed there. Use a charged respawn anchor to set a Nether respawn point instead.
What happens when you use a bed in the Nether?
Attempting to sleep in a bed in the Nether triggers an explosion. It does not provide a sleep or spawn-point benefit, and nearby blocks can catch fire. The blast can kill you, but the result is not inevitable. No reliable universal safe distance is established, so there is no distance you can count on to make testing a bed safe.

How to set a respawn point in the Nether
Use a respawn anchor, Mojang’s intended block for setting a Nether respawn point. Mojang’s guidance says it needs at least one glowstone charge to work. It can hold up to four charges, and each respawn consumes one, so recharge it when needed.

Mojang describes the anchor as a way to set a Nether respawn point in its spawning and dying guidance. Its Java snapshot announcement likewise described it as a block for setting a Nether spawn point, though that 2020 note is historical rather than a complete account of current mechanics: Minecraft snapshot 20w12a.
