Yes. In The Outer Worlds 2, you can change the Commander’s physical appearance after character creation using the Appearance Editor mirror in the Captain’s Quarters aboard the Incognito. The feature was added in Patch 1.2.0.0.
Where to change your character’s appearance
Go to the Captain’s Quarters on the Incognito and use the mirror. Obsidian says the Appearance Editor lets you update the Commander’s appearance whenever you like; its notes do not specify a quest requirement or cost.
What the Appearance Editor changes
Obsidian explicitly names hairstyle, tattoos, piercings, and prosthetics as editable physical features. These are examples from the official description, not a complete list of every setting in the editor.
The patch notes say these edits retain your character’s actions and choices. The feature is for the Commander’s physical appearance, not for changing equipped gear’s visual style or a companion’s look.

How gear and companion visuals differ

Obsidian introduced the Appearance Editor in Patch 1.2.0.0. The official Steam announcements also describe the mirror and the related gear and companion visual options.
