These are the fixes named in the notice; it does not describe a general balance change.
What do App Ver. 1.01.2 and Regulation Ver. 1.01.3 mean?
The notice gives two distinct labels: “App Ver. 1.01.2” for the application and “Regulation Ver 1.01.3” for the regulation file. The application version is therefore 1.01.2, while the corresponding regulation version is 1.01.3; they are not competing names for one version number.
Which platforms got patch 1.01.2?
Bandai Namco listed PlayStation 4, PlayStation 5, Xbox One, Xbox Series X|S, and Steam as target platforms. The official Japanese notice independently confirms the date, version labels, platform list, and update procedure.

How to apply the update and check the regulation file

Download the update for your platform and restart the game to apply it.

On console, log in to the server to download the regulation file.

Check the title screen for Regulation Ver. 1.01.3. If it is not shown, select LOGIN, as Bandai Namco directs.
Applying the update is required for online play.
Could these issues still happen, and is 1.01.2 the latest update?
Bandai Namco cautioned in its official version 1.01.2 notice: “Although this update includes bugfixes, we are aware that similar issues may still occur under some conditions, and we are continuing to work on resolving them.” The fixes should not be read as a guarantee that every similar occurrence was eliminated.
No: 1.01.2 is not the latest update. The official ELDEN RING NIGHTREIGN game page lists later releases, including Hotfix 1.03.5. Check the current in-game version or official news if you need the latest fixes.
