Spoilers ahead. The 2024 Silent Hill 2 remake has eight endings: Leave, In Water, Maria, Rebirth, Dog, UFO, Bliss, and Stillness. Leave, In Water, and Maria are available on a first playthrough; the other five require New Game Plus (NG+). Several outcomes depend on special items or route conditions, so reaching the ending you want is not simply a matter of picking one option at the finale.

Three endings are available during a first playthrough. The other five are tied to NG+, special-item routes, or a prerequisite from a previous run.
Which endings can you get on a first playthrough?
Leave, In Water, and Maria can all be earned without NG+. The available official guidance confirms their first-run availability, but does not publish a definitive scoring formula or exact behavior thresholds for selecting among them. Avoid treating any single action as a guaranteed way to force one of these endings.

Finish a first run before targeting NG+ endings
Use the first playthrough to pursue Leave, In Water, or Maria. If Stillness is a goal, make sure that run ends with In Water: the next-run prerequisite is the prior ending, not simply collecting an item during the same playthrough.
Track NG+ route items
In NG+, keep separate track of items and interactions for Rebirth, Dog, UFO, Bliss, and Stillness. Dog uses Broken Key parts; Bliss is framed by PlayStation as a town scavenger hunt. For exact pickup locations and route sequencing, follow an ending-specific walkthrough rather than assuming the routes share one trigger.
Preserve a manual save before late-game decisions
If you want to pursue more than one outcome in a run, create a manual save before late-game route decisions and keep notes on which items you have collected. This is a practical walkthrough strategy, not an official guarantee that every ending can be produced from one save or one sequence of choices.
Can one ending route override another?
Yes. Special-item interactions can affect which ending takes precedence, so collecting an item does not always mean the route will conclude with the ending you expect. Silent Hill Memories’ route guide notes that Stillness can supersede Rebirth when the relevant item is collected. If you are targeting either one, follow the guide’s specific sequence and verify the intended route before committing to a late-game interaction.
