The documented way to add miscarriage to The Sims 4 is with LittleMsSam’s third-party Miscarriage Chance & Abortion – Miscarriage Part mod. Is there a miscarriage setting in The Sims 4?
The researched route is a third-party mod; the creator’s listing describes the feature as part of LittleMsSam’s Miscarriage Chance & Abortion mod. It is not an EA-supported game setting. What the miscarriage mod does
According to the creator’s listing, a pregnant Sim may receive cramps and pain buffs. The player then takes the Sim to the hospital through the Phone – Travel menu. The pregnancy may continue, or the Sim may receive sad news. These are the creator’s described mechanics; they are not a guaranteed event or independently verified game test.
How the listed chance varies
The mod page gives the following base chances by trimester and age group. These are settings stated by the creator, not independently measured outcomes in gameplay.

The listing says certain negative need buffs can raise the base chance: tired, hungry, and other listed buffs multiply it by two, while exhausted, starving, and other listed severe buffs multiply it by five. Those terms refer to in-game buffs, not real-world health advice.
Optional chance and outcome add-ons
The creator lists optional very-low, low, middle, and high chance add-ons. For example, the page says a first-trimester Elder’s 1.5% base chance becomes 11.25% with the very-low add-on after applying the stated age, negative-need, and setting multipliers. Treat this as the mod page’s example of its configuration, not a promise about observed gameplay.

Other optional files can add the unborn baby to the family tree, enable a chance of death or infertility after miscarriage, or extend the sad or happy mood buff to seven days. Choose only the options you want and read the creator’s current descriptions before adding them.
How to install it and check compatibility
- Open the creator’s CurseForge listing and read the latest file notes and instructions.
- Install XML Injector, which the listing identifies as required.
- Download the main miscarriage file and only the optional add-ons you intend to use.
- Check that the selected file supports your installed game version. At the time of the listing snapshot on October 8, 2026, the page showed a September 1, 2026 release for game version 1.127.41 and marked it Base Game compatible. This can change after game patches, so rely on the current file notes rather than that snapshot.
- Back up your save before changing mods. Before removing files from an established save, check the creator’s current save-related instructions: the listing warns that removing parts can affect babies or deceased text in the family tree.
How to choose the options
- Event frequency: Select among the listed very-low, low, middle, or high chance add-ons based on how often you want the event to occur.
- Family-tree record: Decide whether to use the optional file that adds the unborn baby to the family tree.
- Additional consequences: The listing offers optional death or infertility outcomes; they are not required for the core miscarriage mechanic.
- Mood duration: The creator also lists an option to extend the sad or happy mood buff to seven days.
- Game compatibility: Confirm the exact files you select match your current game version.
Is it an EA or official feature?
No. The mod is made and distributed by a third party. EA’s policy states, “Maxis and EA don’t pre-screen, endorse, or support specific Mods or Custom Content.” The policy also says mods must be non-commercial and distributed free, while allowing a reasonable early-access period before all players receive free access. Passive ads or donation requests may appear on a mod website or distribution site, but not inside the mod. These policy rules do not make an unofficial mod an EA product or an EA-supported feature.
