Spoilers for Red Dead Redemption 2’s main story. No: Arthur’s tuberculosis is a fixed story event, and there is no supported way to prevent or cure it during a normal playthrough. The diagnosis happens in the Chapter 6 mission “A Fork in the Road.” Food, rest, tonics, and other ordinary health management do not reverse the story illness.
Why Arthur becomes “unwell”
Arthur’s tuberculosis is part of the main story, not a temporary status effect or a condition caused by neglecting his health cores. The Chapter 6 mission “A Fork in the Road” is where he collapses in Saint Denis and receives the diagnosis.

If the Wellbeing menu describes Arthur as “unwell,” that warning refers to the story illness. A player discussion about the status connects it to tuberculosis and quotes the description as saying the illness “can not easily be cured” (GameFAQs discussion). It is not a prompt to buy a cure or treat ordinary poison.
Can you avoid the diagnosis by delaying the story?
You can put off the relevant story progression for a time, but that delays the diagnosis rather than preventing it. In a player Q&A, avoiding Strauss’s debt-collection missions is described as eventually blocking ordinary camp and town interactions instead of opening a successful prevention route (Reddit player Q&A). Treat that specific progression behavior as community-reported, not as an official rule. The supported takeaway is that there is no normal story path in which Arthur avoids the illness and continues the main story as usual.

What food, rest, and health items can still do
Ordinary health management remains useful for gameplay, but it is separate from tuberculosis. The game’s health guide explains that Arthur’s Health, Stamina, and Dead Eye cores support their respective attributes; eating and resting help maintain core performance, while temperature and weight affect health and stamina. These systems do not cure the narrative illness.
- Eat and rest to maintain ordinary core performance.

- Do not expect a doctor, tonic, food, or purchasable item to undo the tuberculosis diagnosis.
If Arthur is already diagnosed
Continue managing his ordinary cores for the demands of play, but the story illness remains part of his condition. The available guidance supports no item-based or medical cure that removes it; the diagnosis is a narrative turning point, not a standard ailment to clear from the Wellbeing menu.
