The events can be cleared in either order, but one does not substitute for the other: both are needed to open the way to the Throbbing Domain.
What to bring to each event

How to remove the Wall of Flesh
- Obtain Throbbing Gristle from the first Guillotina progression. It is the required quest item for the Wall of Flesh.
- Carry it to the event at the end of the Caves.
- Choose the placement interaction when prompted. The interaction consumes the Gristle and retracts the flesh.
If the Throbbing Artery quest is not complete, the veins still block the passage after the flesh retracts. Finish the Artery event with Putrid Leech to clear the remaining obstruction.
How to remove the Throbbing Artery
- Obtain Putrid Leech from the second Guillotina progression.
- Carry it to the Throbbing Artery event at the end of the Boneyard, after Dybbuk.
- Choose the attach interaction. If the Wall of Flesh quest is already complete, the veins shrivel and a hole opens.
If you attach the Leech before clearing the Wall, complete the Wall event as well. The pair of item quests opens the Domain route regardless of which event result you see first.

Protect the quest-item carrier
These items impose serious penalties while carried. The in-game description for Throbbing Gristle says, “All units die when they get downed.” Putrid Leech’s description says, “You never level up and die when downed.” Plan the route around the item: protect its carrier, and avoid taking it into a run where your party cannot manage the downside.
- With Throbbing Gristle: a downed unit dies, so avoid risky fights and prioritize keeping the party from being downed.
- With Putrid Leech: its carrier cannot level up and dies when downed, so keep that cat out of danger and consider when to take the item on the route.
A February 21, 2026 walkthrough also advises caution about enemies that can destroy equipment, which could cost the quest item and require another attempt. Treat that as walkthrough advice, not a guaranteed outcome; the route choices and available paths can vary with progression or later patches. For the Wall, the walkthrough describes Alley, Sewers, and Caves; for the Artery, it describes Alley, Junkyard, and Boneyard. Use the current in-game map and choices to confirm your route. What unlocks after both obstacles are cleared
Completing both item quests contributes to opening the Throbbing Domain. Your first visit initially gives access to the Meat Altar, not the full area. To open the rest, take Guillotina’s Head—earned from the third Guillotina progression—to the altar and complete its quest.
