No. In the Iki Island side tale “The Impact of Loss,” Jin can rescue either Doshun the monk or Togo, Hotoke’s husband. The other captive dies, according to gameplay guides; clearing enemies on both sides does not save both.
What happens in “The Impact of Loss”?
The tale puts Doshun and Togo in danger and asks you to choose which captive to rescue. The available walkthrough reporting says there is no outcome in which both survive.

Which character should you rescue?
Choose based on which outcome you prefer. A gameplay guide reports that rescuing Doshun brings him back to camp with Hotoke. Rescuing Togo means Doshun dies and Hotoke becomes ill. These branch details come from a secondary guide, not an official quest guide.
The same guide reports identical quest rewards for either choice, so it identifies no reward advantage to rescuing one captive over the other.

Does defeating enemies on both sides save both?
No. A guide that discusses clearing enemies on both sides still says the other prisoner dies. The sources do not establish a combat trick or alternate route that lets Jin rescue both.

“The Impact of Loss” is part of Iki Island. PlayStation’s Director’s Cut announcement describes Iki Island as a new story expansion included with that edition. PlayStation’s product page lists a PS5 disc edition that includes the expansion. If you already own an edition that includes Iki Island, you do not need another copy to make this choice.
