The “Karsus book” is The Annals of Karsus, and letting Gale read it opens a conversation about his plans for the Crown. Reading it does not, by itself, decide his ending. If you refuse companion Gale twice when he asks to read it, he loses 20 approval.
What happens if Gale reads The Annals of Karsus?

When Gale reads the book, he learns about the Crown of Karsus and Karsus’s attempt to supplant the goddess of magic. That leads to a conversation in which you can encourage Gale’s ambition or challenge it. The book sets up the Crown question; it is not the final choice about what Gale will do.
Let Gale read it or refuse?

No. Reading The Annals of Karsus reveals the Crown’s original designs and triggers a discussion about what Gale wants to do with it. The Annals of Karsus is the Act Three book tied to Gale’s Crown storyline. The Necromancy of Thay is a separate Act One book associated with Search the Cellar and Unlock the Ancient Tome. Its effects and later interactions—including the Tharchiate Codex—do not apply to The Annals of Karsus. Gale can also consume the Necromancy of Thay, destroying it without its benefits.
