Bethesda’s Fallout 4 support page says a previously reported Xbox achievement issue was resolved on April 19, 2024. The official sources available here do not confirm that Bethesda is currently investigating a new issue. If an achievement is missing from your Xbox profile, check its requirements, whether mods were enabled on the save, and whether your Xbox account needs to resynchronize.
Is there a current Fallout 4 achievement outage on Xbox?
Bethesda documents an Xbox achievement issue for Fallout 4 that it says was resolved on April 19, 2024. That historical notice does not establish whether Xbox achievement tracking is healthy today, and the official sources cited here do not confirm a current Bethesda investigation. An achievement failing to appear can still result from game requirements, a connection delay, or the save’s mod status.
Bethesda’s Fallout 4 support article also notes that a missing achievement may not appear immediately because of networking or connection problems. Its service-status portal lists Bethesda services such as the Fallout 4 Creation Club Store and Mods; that is not a report on Xbox achievement tracking.
What to check when an achievement is missing
- Confirm the achievement’s conditions. Review the specific requirements and make sure every condition has been met. An achievement may have more than one requirement.
- Allow for a connection delay. If you have met the conditions, Bethesda says networking or connection problems can delay the achievement appearing.
- Refresh Xbox account synchronization. Sign out of your Xbox account, reboot the console, then sign back in. Bethesda recommends this sequence to refresh the achievement list. See its Xbox achievement troubleshooting steps.
- Retry the achievement. If it still has not appeared after the account refresh, try meeting or triggering its requirements again.
- Contact Microsoft if it remains unresolved. Bethesda says its support team cannot manually unlock achievements and directs players to Microsoft Support for persistent Xbox issues.
Could mods be preventing Fallout 4 achievements?
Yes. Bethesda’s published Fallout 4 mod policy says enabling a mod disables achievements and trophies for that game state. For achievement play, Bethesda instructs players to disable all mods and load a save from before mods were enabled. Disabling mods on a save that already had them enabled does not replace the need to return to that earlier save.

For a persistent Xbox achievement-tracking problem, contact Microsoft Support. Bethesda’s Xbox troubleshooting guidance says Bethesda Support cannot grant or manually unlock achievements, so it cannot resolve a missing unlock by adding it to your account.
