Who is Lena?
Lena is a human member of the Outsiders, a community living beyond the jurisdiction of the COG. She lives in Riftworm Village, a settlement built inside the remains of a Riftworm. The character reference lists Gears 5 as her first appearance. Character reference

What happens to Lena in Gears 5?
After Oscar Diaz becomes village chief and imposes stricter rules to protect the settlement from the Swarm, Lena leaves with Norsko and Freddie. The three set up camp in Kadar Valley, but their attempt to survive there falls apart: a DeeBee kills Freddie, and Norsko is later captured and transformed into a Juvie. Lena continues alone and takes shelter in a substation. When it loses power, she dies from the cold. Character reference
Where do you find Lena?
During the Lost Outsiders side mission, look for Lena in the East Tower Substation in Kadar Valley. A nearby Security Memorandum contains the access code for the North Tower Substation; it is a separate collectible clue, not the location where Lena is found.
