Usually, the cause is a setting or an interaction that cannot cause pregnancy: WickedWhims pregnancy may be disabled, the selected chance may be 0%, a Sim may not be in the fertile window, or birth control may be in use. First confirm the pregnancy mode and interaction, then check cycle timing, birth control, and Sim eligibility. Use WickedWhims’ in-game Pregnancy Test or Pregnancy Info to verify before assuming an attempt failed.
Start with the interaction and pregnancy setting
WickedWhims documents vaginal or climax sex as interactions that can cause pregnancy, unless pregnancy is disabled in the mod’s Pregnancy Settings. Other types of sex do not necessarily lead to pregnancy. Check the selected mode before troubleshooting anything else: WickedWhims’ pregnancy documentation describes three modes.

If you use Simple mode, check both chances
In Simple mode, a 0% chance prevents pregnancy for the affected role. For a player-Sim/NPC pairing, the documented settings are separate: Player Sims Pregnancy Chance and NPC Sims Pregnancy Chance. Make sure both are above 0% for pregnancy between those roles. This mode does not use menstrual cycles or fluctuating fertility, so cycle timing is not the explanation when Simple mode is selected.
If you use Menstrual Cycle mode, check timing
Each Sim has an individual cycle in this mode, and pregnancy depends on being in the fertile window. To inspect the relevant information, Shift-click the Sim and open the Wicked menu’s Pregnancy Info. A toilet can also be used for a Fertility Awareness Test.

WickedWhims documents these cycle-length ranges by selected cycle mode. They are gameplay ranges, and the documentation does not state a publication year:

Fertility settings can also affect the chance of pregnancy. In the mod’s documented Realistic setting, the impregnating Sim’s fertility chance is 91% for Young Adults, 85% for Adults, and 75% for Elders. In Boosted, it is 96%, 90%, and 80%, respectively; in Extreme, it is 100% for all three. These are WickedWhims gameplay values, not guarantees that a particular encounter will result in pregnancy.

Check whether birth control was used
WickedWhims’ pregnancy guide says birth-control auto-use is enabled by default and can be changed in Pregnancy Settings. Check whether condoms or pills are being automatically used, and whether the relevant items are in the appropriate Sim’s inventory. The outcome also depends on the configured accuracy mode.
For the mod’s Realistic birth-control setting, the documented failure chance is 5% for condoms and 9% for birth-control pills. The documentation says pills need to be taken for three days to reach full effect. These are mod mechanics, not real-world health information.

Confirm the Sim can become pregnant
WickedWhims chance settings are separate from base-game pregnancy eligibility. EA’s The Sims 4 Pregnancy Guide says young adult and adult Sims can be configured in Create a Sim to become pregnant, get others pregnant, or neither. EA says only female Sims can become pregnant by default. If the Sim’s base-game settings do not allow them to become pregnant, changing WickedWhims’ chance alone does not resolve that eligibility issue.
Allow time for discovery and use the mod’s checks
A pregnancy may not be visible immediately after an interaction. WickedWhims documents that discovery can occur 24 Sim-hours later when the discovery behavior is enabled. Use the in-game Pregnancy Test or Pregnancy Info to check; WickedWhims cautions that external mods used to verify pregnancy may report incorrect information.

If it still does not work, verify versions and report the issue
Game and mod updates can affect compatibility, but a past fix does not establish that a current save has the same problem. EA’s July 16, 2025 update said it reverted performance optimizations associated with pregnancy issues reported after the July 1 update. The note identified PC version 1.116.202.1030 as the July 1 version and 1.116.232.1030 as the fixed PC version. It also lists Mac and console versions. EA said, “Any Sim pregnancy related mods should return to their pre-update functionality, though you should still check with the specific mod author for updates.” Treat those version numbers as historical patch information, not a diagnosis of a current issue. - Compare the installed game version with current official information and check the WickedWhims download page for the mod version and compatibility information.
- If contacting WickedWhims support, provide the game version, mod version, steps that reproduce the issue, and relevant exception files, as requested on the WickedWhims support information page.
Without the save’s settings, installed versions, and other pregnancy-related mods, there is no basis to identify a particular conflict. Check the documented settings first, then include the requested details if you ask for help.
