SU-42481-9 means the PS4 system-software update failed. Start with a restart, an Ethernet connection, and deletion of the incomplete update from Notifications. If the console will not reach the home screen, use Safe Mode. Only use Initialize PS4 (Reinstall System Software) as a last resort: Sony says that process erases data stored on the console.
What SU-42481-9 means
Sony defines SU-42481-9 as a failed PS4 system-software update. The code does not, by itself, prove that the hard drive, Blu-ray drive, motherboard, or network is defective. It can appear after a failed download, during an update loop, or when the console falls back to Safe Mode.
The safest order is to retry without deleting local data, then use Safe Mode and the standard USB update file. Reinstallation is a separate, destructive procedure.
If the PS4 still reaches the home screen
- Back up important saves and other data if the console is working normally. Do not assume a later reinstallation will preserve anything stored locally.
- Connect the PS4 directly to your router with an Ethernet cable if possible. Wired networking removes Wi-Fi signal and interference variables, although it cannot repair damaged update media or hardware.
- Open Notifications from the function area.
- Highlight the failed system-software update, press the controller’s OPTIONS button, and choose Delete.
- Open Settings > System Software Update and try the download again. Sony’s error-code instructions are at this support page.
If the second attempt fails, stop repeatedly downloading the same file and move to Safe Mode or a USB update.

If the PS4 cannot reach the home screen: use Safe Mode
- Turn the PS4 completely off.
- Press and hold the power button. Release it after the second beep.
- Connect a DUALSHOCK 4 controller with a USB data cable. A charge-only cable may not communicate with the console.
- Press the controller’s PS button.
- Choose Update System Software. Try the online update first if the console can connect.
Sony’s Safe Mode procedure is documented at its Safe Mode guide. If the online attempt fails, use the standard update file from USB. Do not select the reinstallation option merely because an ordinary update failed.
Update PS4 system software from a USB drive
Prepare the standard update file
- On a PC or Mac, format a USB drive as FAT32 or exFAT.
- At the USB drive’s root, create a folder named
PS4. - Inside it, create a folder named
UPDATE. - Download Sony’s clearly labeled PS4 update file from the official PS4 system-software page.
- Place it in the
UPDATEfolder with this exact name:PS4UPDATE.PUP.
USB root/
└── PS4/
└── UPDATE/
└── PS4UPDATE.PUP
Use the standard update download for an update attempt. Sony also offers a separate reinstallation download with the same filename; that file belongs to the destructive reinstall procedure below.

Install the update
- Insert the prepared USB drive into the PS4.
- Start Safe Mode by holding the power button until the second beep, then connect the controller by USB and press PS.
- Select Update System Software > Update from USB Storage Device > OK.
Use only update data downloaded from Sony. Do not turn the console off during installation, and avoid starting an update immediately before or after a power outage, as Sony warns that interruption can damage the process.
If the PS4 cannot find PS4UPDATE.PUP
- Confirm the drive is FAT32 or exFAT.
- Check that
PS4andUPDATEare folders directly on the USB root, not nested inside another folder. - Check the exact uppercase spelling:
PS4UPDATE.PUP. - Remove accidental extensions such as
.PUP.pup,.PUP.zip, or.PUP.txt. - Make sure you downloaded Sony’s official file and placed it inside
UPDATE, not beside it. - Use a known-good USB data cable for the controller.
- As practical troubleshooting, try another USB port or another compatible drive if the structure and file are correct.
Do not choose a download based only on its file size. Select Sony’s update or reinstallation button that matches the Safe Mode option you intend to use.

Last resort: reinstall PS4 system software
Warning: Initialize PS4 (Reinstall System Software) deletes data stored on the console, including local saves, installed games, screenshots, settings, and other user data. Back up first if the PS4 can still reach the home screen. Cloud-save access depends on your account and service setup, and a console stuck before the home screen may not be able to create a backup.
Consider reinstallation only after restarting, deleting the failed download, trying Ethernet, using Safe Mode updating, and attempting the standard USB update. It may be appropriate for a persistent update loop, a console that explicitly requests reinstallation, or a procedure Sony Support directs you to follow.

This rebuilds the system-software installation and returns the console to its initial software state; it is not a data-preserving update.

If a correctly prepared official USB update and, only when justified, reinstallation both fail, possible explanations include unstable networking, corrupt media, an incompatible or damaged USB drive, a failing internal hard drive, or another hardware fault. The error code alone cannot distinguish among them. Stop repeating random procedures and use PlayStation Support or Sony’s regional repair diagnostic path. Repair eligibility, warranty coverage, shipping, and pricing vary by country, model, and date.
