Where Requiem begins

That setup lets returning players continue the same story while giving newcomers enough immediate context to understand the crisis. Xbox Wire described Requiem as a continuation of Innocence that can still welcome people who did not play the earlier title. The emotional stakes, however, are clearer if you know what Amicia and Hugo endured previously.
What the Macula is
The safest canon description is the one used by Xbox Wire: the Macula is “the disease which affects Hugo.” In practical story terms, it is the affliction that connects Hugo to the rat plague and to the extraordinary abilities he experiences. When the condition seems quiet, the danger recedes; when it flares, the rats and the destruction associated with them return.
Characters sometimes call Hugo’s condition a curse, but “curse” describes how frightening and inescapable it feels rather than supplying a complete medical or supernatural definition. The official material does not settle every question a player might ask about the Macula. The reviewed summaries do not establish its exact origin, a complete inheritance model, precise thresholds for each phase, or whether it should be treated as a thinking entity. Those points need to be read from the finished game rather than filled in with fan explanations.
What is confirmed at a high level
- Hugo is the person affected by the Macula in Requiem.
- His condition can appear controlled for a time.
- His powers reawaken during the story.
- The return of those powers is associated with rats and renewed destruction.
- Finding a way to save Hugo drives the journey to the south and ultimately toward La Cuna.
This is enough to understand the plot without pretending that the publisher’s synopsis is a complete encyclopedia. The Macula is not simply a synonym for “the rats”: it is Hugo’s disease, and the rats are one of its most visible and destructive consequences.
Why Hugo and Amicia go to La Cuna
Once Hugo’s condition worsens, the siblings need more than temporary control. They leave their devastated home and travel south, placing their hope in an island described in prophecy. That island is La Cuna. The official premise presents it as a possible key to saving Hugo, not as a guaranteed cure.

Amicia’s decision is therefore practical and emotional at the same time. She is trying to keep Hugo alive, but she is also trying to protect his chance of having an ordinary life. The journey gives the siblings a destination when every familiar institution has failed them. It is an act of hope under extreme pressure, not evidence that they already understand the Macula.
Several questions commonly asked by players go beyond what the short official descriptions confirm. The summaries do not explain the complete history of La Cuna, the meaning of every symbol or vision, or the precise cause-and-effect linking the island to Hugo’s condition. Those answers belong to the game’s later dialogue, records and scenes.

What La Cuna is (without late-game spoilers)
In spoiler-free terms, La Cuna is a Mediterranean island associated with a prophecy about a Carrier and a Protector. It is presented as a place that may contain knowledge or circumstances capable of helping Hugo. The island is not introduced as a simple safe haven: the promise surrounding it becomes part of the tension, because Amicia and Hugo must discover whether the prophecy can actually deliver what they need.
The setting also changes the tone of the adventure. Instead of remaining in the devastated landscapes familiar from the first game, the story moves into a warmer southern environment. That contrast matters: bright summer scenery and the beauty of the Mediterranean sit beside plague, violence and grief.

Major spoilers: what the La Cuna storyline resolves
Continue only if you have completed the relevant sections of Requiem. The island storyline uses the Carrier-and-Protector prophecy to frame Hugo’s condition and the choices made around him. It also makes clear that a hopeful destination cannot remove the moral and emotional cost of living with the Macula. The late story is less a tidy explanation of a disease than a confrontation with what Amicia, Hugo and the people around them are willing to do to postpone loss.

La Cuna’s revelations should be read through the game’s evidence rather than through a single slogan. Conversations, written records and the island’s events provide context that the marketing synopsis intentionally withholds. If you are checking a particular claim about the island’s past, the reliable test is whether the game states it directly or shows it unambiguously; a visual resemblance or an interpretation of a vision is not automatically a confirmed rule.
The story’s resolution also explains why the search for a cure cannot be separated from the siblings’ relationship. Hugo’s powers affect the world around him, but the decisions about how to respond remain human decisions. That is why the ending’s meaning depends on the player’s understanding of protection, responsibility and grief, not only on a timeline of Macula events.
Does the Macula control the rats?
Requiem clearly connects Hugo’s reawakened affliction with the return of rats, and Hugo can interact with the swarm in ways ordinary people cannot. That supports saying the Macula gives him a special relationship to the plague. It does not, by itself, justify every stronger formulation of the question.
“Control” can mean several different things: summoning rats, directing their movement, sensing them, or being part of a larger cycle that the carrier does not fully command. The official descriptions do not define those distinctions. Use the game’s specific scenes and dialogue when you need to explain a particular ability, and avoid treating a broad marketing description as a complete mechanics manual.

Game director Kévin Choteau discussed the spectacle of the rat systems by citing “300,000 rats on the screen.” That is a developer-reported rendering figure from a 2022 interview about the Xbox Series X|S version, not a real-world population statistic or an independently verified performance benchmark.
Setting: Provence, the Mediterranean and historical inspiration
The journey’s geography is deliberate. Choteau described the sequel as exploring southeastern France, Provence and the Mediterranean. He identified Roman, Moorish and Byzantine influences, while contrasting summer’s warm, colorful landscapes with the brutality taking place within them. The visual contrast is part of the game’s identity: beauty does not cancel suffering, and a bright landscape can still be dangerous.
Lead writer Sébastien Renard described research using books, online resources and advice from Roxane Chila, a doctor in Medieval History, alongside the team’s familiarity with parts of France. He also stressed that the game is historically inspired fiction rather than a documentary reconstruction.
Where the game deliberately departs from history
- Women in the game have more freedom than would generally have been realistic for the period.
- Dialogue dynamics are written for dramatic clarity, not as a transcript of medieval speech.
- Amicia’s trousers are an intentional anachronism.
These choices do not invalidate the setting research. They tell you how to use it: the regions, architecture and cultural references create a believable world, while the characters and plot are shaped for a modern adventure game.
The themes behind the lore
Choteau described the game’s central themes as death and loss, but also as an “ode to life.” His full explanation was: “And Requiem, if it’s a funeral song, it’s also an ode to life, it’s closing a chapter to start something, like going to a funeral and saying goodbye to someone while preparing for a new day to begin.” The statement is the director’s interpretation of the game’s themes, not an in-world explanation of the Macula.

That framing helps explain why the story spends so much time on ordinary moments amid catastrophe. The island quest is about survival, but it is also about what kind of life the characters are trying to preserve and what they may have to surrender. Reading every event only as a clue to the Macula’s mechanics misses the deliberate emphasis on attachment and mourning.
Newcomer guide: how to follow the story
- Remember the starting point. Amicia and Hugo have survived the first game and defeated Vitalis; the sequel begins six months later.
- Track Hugo’s condition. The apparent control at the start is temporary, and the return of his powers triggers the new crisis.
- Separate the disease from its symptoms. The Macula is Hugo’s affliction; rats and destruction are consequences associated with its activity.
- Treat La Cuna as a possibility. The prophecy gives the siblings a reason to travel south, but the island is not advertised as a guaranteed cure.
- Read the island’s evidence in context. Use dialogue, documents and scenes to distinguish confirmed lore from an interpretation of imagery.
Following those five threads is enough to understand the emotional and narrative spine even if you did not play Innocence. Returning players will recognize additional callbacks, but the core motivation remains explicit: Amicia is trying to save Hugo while confronting what his condition means for both of them.
What is established, and what remains open
The strongest confirmed picture is deliberately compact. Hugo suffers from the Macula; the condition can seem controlled and then return; rats and destruction accompany its resurgence; and a prophecy sends the siblings toward La Cuna in search of help. Beyond that outline, the game asks you to assemble meaning from what its characters discover.
That approach is useful when discussing lore online. Present direct statements as canon, identify interpretations as interpretations, and do not turn an unanswered question into a rule simply because it produces a neat theory. The official material supports the premise and themes; it does not replace the game’s complete text.
