The coordinates below use the in-game map convention X, Y and are grouped by zone; coordinate data comes from the community-maintained Occult Record reference, not an official Square Enix coordinate list.

How to access the Occult Record and explore
Open the Duty menu and choose Collection to view Occult Record entries. Square Enix says the record gains information as you progress through the story, discover notes, and access certain areas; it is not just a checklist of survey coordinates. The community reference also notes that some entries come from quests or notes, so finding every point does not necessarily complete the record.

Patch 7.25 introduced South Horn. Entry requires a level 100 Disciple of War or Magic and completion of “The Phantom Village”; speak to Jeffroy in the Phantom Village to enter. Patch 7.55 added North Horn and new Occult Record entries, with South Horn quest completion required to unlock the Occult Record. Mounts can be ridden in the Occult Crescent, but flying is disabled. As Square Enix puts it in the Patch 7.25 notes, “Players are free to ride mounts at any time after entering the Occult Crescent, but they cannot fly.”

The coordinates above cover the 14 listed South Horn survey points and 14 listed North Horn survey points, plus the point in Forked Tower: Blood. They do not represent every possible Occult Record entry: story progression, notes, and access to areas also add information. The named coordinates are from the community-maintained reference; Square Enix’s patch notes establish the zones and record system, but do not publish this coordinate table.
