Find all three in a region to reveal its Primordial Vault. The checklist below gives each fragment’s subregion and the landmark or access action to look for; several are hidden behind grates or require climbing, grappling, or gliding.

Location guides sometimes differ on the precise object or route at a landmark. Use the named subregion and access cue to orient yourself rather than treating one approach as mandatory. PowerPyx’s guide, published September 11, 2025, lists the nine fragments across these three regions; it reports no Vault Fragment set in Dominion. The Fadefields: three fragments
Coastal Bonescape
Go to Stillshore and search the southwest, coastal side. The fragment is around a beachside house or building; check the terrace and nearby box or crate.
The Howl
Look for the cave northeast of The Launchpad. The north side of the mountain is a useful approach, and the fragment is farther into the cave rather than at its entrance.

Idolator’s Noose
At Rosemary’s Reserve, look for a grate or ventilation panel. Pull it away to access the hidden crawlspace or room beneath the structure.
Carcadia Burn: three fragments
Lopside
The fragment is on top of a fuel or gas station. Climb or grapple onto the roof, then check the table or box there.

Tonnage Peel
At the Wreck of the Last Great Hope, find the floor grate or vent cover in the wreck or tunnel. Pull it open and drop into the concealed area below.
Ruined Sumplands
Search the waterfront at the base of a hill, among the rocks near Hull’s Handbasket.

Terminus Range: three fragments
Cuspid Climb
This fragment is in a cave high on a cliff or mountain. One route described by guides glides toward a platform by the cave; another reaches it via the Reliquary silo. The destination is the same cave, so use whichever approach is accessible from your position.
The Low Leys
Near The Pit, climb the rocky slope to a high ledge and use the grapple point to reach the fragment.

PowerPyx describes each vault as three arenas of enemy waves followed by a Vault Guardian encounter, with access to a permanent power-up after the guardian is defeated. These are the guide’s reported rewards and sequence; the cited location guides do not establish whether every route detail has remained unchanged in later game builds.
