There is no official, permanently correct best class in Animal Hospital Anomaly. The safest all-around shortlist is Surgeon or Head Nurse, while Secret Agent is the specialist choice for players who value combat-oriented anomaly handling. Doctor and Psychologist are sensible middle-ground candidates, and lower-cost roles can be better when your priority is learning the shift, supporting sanity, or filling a team job. Treat every ranking as provisional: third-party guides disagree, and the live October 2026 roster, costs, and balance changes still need to be checked on the in-game class screen.
Provisional tier list
The table below combines the two detailed June 2026 guide rankings without pretending they are an official consensus. It is an editorial way to understand the current discussion, not a developer-published leaderboard.

| Tier | Classes | Best fit | What the placement means |
|---|---|---|---|
| S+ / S | Head Nurse, Surgeon, Secret Agent | Players seeking broad value or strong anomaly-focused play | Head Nurse is placed S+ by one detailed guide and S by another; Surgeon is S in both detailed discussions; Secret Agent is S in one and A in another. |
| A | Doctor, Psychologist | Players who want useful support without committing to a narrow specialist | Both are placed A by the more detailed guide. The other June list does not rank them in its published excerpt. |
| B | Security, Paramedic | Players filling a practical team role | The detailed guide places both in B, below the broad-utility and premium combat picks. |
| B / C | Secretary | Players prioritizing access, utility, or a lower barrier to entry | One list places Secretary in B; the detailed list places it in C, showing how much criteria change the result. |
| C | Intern | New players learning the game or accepting limited specialization | The detailed guide places Intern in C. No independent performance data establishes a universal position. |
| D | Nurse | Players choosing for personal preference rather than maximum reported efficiency | The detailed guide places Nurse in D; this is an editorial judgment, not proof that the class cannot succeed. |
If you need one short answer, start with Surgeon for a free-to-play all-rounder, consider Head Nurse if its premium unlock is available and its current kit suits you, and choose Secret Agent when hostile encounters are the part of a shift you most want to optimize.

Tier lists measure different things. A guide updated in June 2026 puts Surgeon and Head Nurse in S, Secret Agent in A, and Secretary in B. A separate, more detailed guide puts Head Nurse in S+, Surgeon and Secret Agent in S, Doctor and Psychologist in A, Security and Paramedic in B, Secretary and Intern in C, and Nurse in D. Neither list is an official statement from the game’s developers.

The disagreement is useful because it exposes the real decision. A player who values sanity efficiency may rank a support class above a combat specialist. Someone who mostly plays alone may value inventory utility and recovery options more than a coordinated group that already has a support player. Unlock cost can also move a class up or down for a new account.
Community discussion reaches the same general conclusion: players ask for the best class but evaluate classes through their own shift patterns. That conversation is anecdotal, not controlled performance evidence, so it should inform questions to ask rather than settle the ranking.
The five criteria that matter more than a letter grade
Sanity support
Check whether the class helps restore sanity, preserve it, or reduce the cost of recovering after a difficult encounter. Record the effect shown in the current class description rather than relying on an old tier graphic. A class that prevents failed shifts can be more valuable than one with a higher theoretical ceiling.
Anomaly handling
Look for a reported tool, resistance, or encounter advantage that changes how you deal with dangerous anomalies. Secret Agent is described in the available guides as a premium, combat-oriented choice, but the exact current effects and numbers were not independently verified. Confirm the live description before spending currency.
Inventory and supply access
Inventory utility includes carrying capacity and the ability to reach or manage supplies during a shift. This axis is easy to undervalue: fewer return trips can improve both solo consistency and team uptime, even if the class has no obvious combat bonus.
Unlock cost
Cost changes the practical tier. A powerful role that requires premium currency may be a poor first purchase if you are still learning the map, while a free or Animal Coin role can deliver more value to a new player. The guides report currency categories, but not a verified current price list.

Note: This device is not compatible with Linux.
- What You Get: Anker USB-C Hub (5-in-1, 4K HDMI), welcome guide, 18-month warranty, and our friendly customer service.
Playstyle compatibility
Ask whether you play alone, with a fixed group, or with changing teammates. A class that looks average in a solo ranking may be excellent when it fills the one job your group lacks. Conversely, stacking similar support roles can leave a team without enough anomaly control or carrying capacity.

Surgeon
Surgeon is the clearest free-to-play recommendation in the available material. One June 2026 tier page places it in S and explicitly describes it as a free-to-play pick; the more detailed guide also places it in S. That combination makes Surgeon the default starting point for players who want broad value without a premium unlock.
Choose Surgeon when you want flexibility and do not yet know which axis will matter most to you. Before treating it as an automatic best class, verify its current sanity, anomaly, and inventory effects in the live client. The evidence supports its all-around reputation, not a guarantee that it wins every shift or every matchup.
Head Nurse
Head Nurse receives the strongest placement in the detailed guide, which rates it S+, while the other June list places it in S and describes it as a premium pick. It is therefore the leading candidate for players willing to use premium currency and looking for broad utility rather than a narrowly defined specialist.
Premium status alone is not proof of superior value. Confirm the current unlock availability, price, and class description in-game, then compare the role with Surgeon using your own priorities. If you mostly play with a group that already has its support needs covered, a different class may produce more practical value.

Secret Agent
Secret Agent is the most contested high-tier specialist. The detailed guide rates it S, while the other list rates it A. Both accounts identify it as a premium, combat-oriented option, making it the natural candidate for players who prioritize hostile anomaly encounters.
Rank #3
- Sleek 7-in-1 USB-C Hub: Features an HDMI port, two USB-A 3.0 ports, and a USB-C data port, each providing 5Gbps transfer speeds. It also includes a USB-C PD input port for charging up to 100W and dual SD and TF card slots, all in a compact design.
- Flawless 4K@60Hz Video with HDMI: Delivers exceptional clarity and smoothness with its 4K@60Hz HDMI port, making it ideal for high-definition presentations and entertainment. (Note: Only the HDMI port supports video projection; the USB-C port is for data transfer only.)
- Double Up on Efficiency: The two USB-A 3.0 ports and a USB-C port support a fast 5Gbps data rate, significantly boosting your transfer speeds and improving productivity.
- Fast and Reliable 85W Charging: Offers high-capacity, speedy charging for laptops up to 85W, so you spend less time tethered to an outlet and more time being productive.
- What You Get: Anker USB-C Hub (7-in-1), welcome guide, 18-month warranty, and our friendly customer service.
Its lower placement in one list does not mean it is weak; it reflects the difference between specialist power and all-around utility. Secret Agent is a logical choice for experienced players who understand encounter risks, but new players should first confirm that the current combat advantages justify the premium unlock in their usual shift pattern.
Doctor
The detailed guide places Doctor in A. That is a strong middle tier, suitable for players who want a capable role without choosing one of the three most debated top picks. The available material does not establish a current numerical bonus or a complete ability list, so use the live class panel to determine whether Doctor’s value is concentrated in sanity, recovery, anomaly response, or another area.
Doctor is best evaluated against the specific gap in your team. If another teammate already covers the same function, a class with inventory or encounter utility may improve the group more.
Psychologist
Psychologist is also placed in A by the detailed guide. That positioning makes it a reasonable candidate for players who put sanity management near the top of their priorities. It should not be read as proof of a particular restoration rate or passive effect; those figures were not established in the available evidence.
Compare Psychologist with Surgeon or Doctor by observing which part of a shift causes failures. If sanity loss is the limiting factor, a support-oriented role can outperform a higher-ranked combat pick for your account even when the global tier label says otherwise.
Security
Security is placed in B by the detailed guide. A B placement usually signals useful situational value rather than an unusable class. Security can make sense for a coordinated team that needs a dependable role dedicated to operational safety, provided the current class description confirms that it addresses your group’s weakness.

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
Do not infer exact defensive statistics from the class name. Check the current tooltip and test the role in a low-risk shift before committing currency or rebuilding your team around it.

Paramedic
Paramedic shares the detailed guide’s B tier with Security. It is therefore a practical, situational choice rather than a universal recommendation. Players should compare its current recovery or support function with Doctor and Psychologist, especially if the team already has a dedicated sanity specialist.
The evidence does not provide verified cooldowns, ranges, or healing values. Treat any older video or tier graphic as potentially stale until the in-game description confirms it.
Secretary
Secretary demonstrates why tier lists should not be merged mechanically. One June list places Secretary in B, while the more detailed guide places it in C. That split may reflect different weighting of inventory and access utility versus anomaly performance.
Secretary can be a sensible choice when your priority is logistical efficiency or when you want a lower-pressure role while learning the hospital. It is not a safe pick to label universally strong or weak; decide after checking how much its utility reduces travel, downtime, or supply problems in your own shifts.
Intern
The detailed guide places Intern in C. For a new player, a lower tier does not automatically make Intern a mistake: learning routes, anomaly cues, and resource timing may matter more than squeezing out a theoretical advantage. Use Intern if the unlock is accessible and you are still building fundamentals, then reassess after you can identify the bottleneck in your runs.
Best Value
- 5-in-1 Connectivity: Equipped with a 4K HDMI port, a 5 Gbps USB-C data port, two 5 Gbps USB-A ports, and a USB C 100W PD-IN port. Note: The USB C 100W PD-IN port supports only charging and does not support data transfer devices such as headphones or speakers.
- Powerful Pass-Through Charging: Supports up to 85W pass-through charging so you can power up your laptop while you use the hub. Note: Pass-through charging requires a charger (not included). Note: To achieve full power for iPad, we recommend using a 45W wall charger.
- Transfer Files in Seconds: Move files to and from your laptop at speeds of up to 5 Gbps via the USB-C and USB-A data ports. Note: The USB C 5Gbps Data port does not support video output.
- HD Display: Connect to the HDMI port to stream or mirror content to an external monitor in resolutions of up to 4K@30Hz. Note: The USB-C ports do not support video output.
- What You Get: Anker 332 USB-C Hub (5-in-1), welcome guide, our worry-free 18-month warranty, and friendly customer service.
Nurse
Nurse is the only listed role placed in D by the detailed guide. That is the guide author’s lowest comparative placement, not evidence that successful play is impossible. A familiar class can still be the right choice if its current kit matches your preferred shift routine or if its unlock conditions make it the only practical option.
Because the available material does not include controlled tests, do not convert the D label into a claim about failure rate or damage output.

| Your priority | First classes to inspect | Reasoning |
|---|---|---|
| Free-to-play, broad utility | Surgeon | One June guide identifies Surgeon as free to play and places it in S; the detailed guide also rates it S. |
| Premium, broad utility | Head Nurse | Head Nurse is S+ in the detailed guide and S in the other list, but current price and availability require in-game verification. |
| Combat-oriented anomaly play | Secret Agent | The guides describe it as a premium combat-focused option, with placements ranging from A to S. |
| Sanity management | Psychologist, Doctor, Surgeon | These are the candidates to compare in the live descriptions; no verified restoration figures were supplied. |
| Team safety or recovery | Security, Paramedic, Doctor | The detailed guide places Security and Paramedic in B and Doctor in A; select according to the gap your team actually has. |
| Logistics and access | Secretary, Surgeon, any class with confirmed inventory utility | Secretary’s B-versus-C split shows that logistical value is weighted differently by different guides. |
| Learning with minimal commitment | Intern or the least expensive available role | Practice can be more valuable than a nominal tier while you learn routes and encounter timing. |
Unlock currency and purchase decisions
One detailed guide reports Head Nurse and Secret Agent as Robux unlocks and presents the other listed roles generally as Animal Coin unlocks. The retrieved material does not establish official current prices, regional differences, discounts, or whether every role remains available on the same terms.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsUse this purchase sequence:
- Open the live class or unlock screen and confirm that the role is currently available.
- Record the displayed currency and exact price for your account; do not rely on an old guide graphic.
- Read the current ability text, including restrictions, cooldowns, and inventory effects.
- Compare the cost with how often you play solo and whether your regular team already covers the same function.
- Buy only after identifying the shift problem the class is expected to solve.
The roster may be larger than the familiar ten-class list
Class count is unsettled. One class reference says it lists ten classes and was updated June 30, 2026. Other search results report an eleven-class roster that includes Firefighter, followed by later-reported additions such as Scientist and Barista. No official patch-note confirmation was available for those claims in the material reviewed here.
That uncertainty matters because a tier list can become incomplete without becoming obviously wrong. Before choosing from an older chart, scroll through every class currently visible in your client. If Firefighter, Scientist, Barista, or another role appears, evaluate it with the same five criteria instead of forcing it into an outdated tier.
A five-minute method for choosing your own main
- Define the failure point. Decide whether runs usually end because of sanity loss, dangerous anomalies, missing supplies, slow movement between tasks, or a team composition problem.
- Filter by currency. Remove roles you cannot or do not want to unlock, then compare the remaining descriptions.
- Check solo versus team use. Solo players generally need self-sufficiency; coordinated groups can specialize and trade coverage.
- Read every current tooltip. Note explicit effects and limitations. Do not infer an ability from the class name.
- Run a controlled comparison. Use the same shift type and similar route for two or three attempts, recording sanity stability, anomaly outcomes, supply trips, and objective completion.
- Recheck after updates. A tier label from June 2026 may not reflect an October 2026 balance change or a newly added class.
Solo and team recommendations
Solo players
Solo play rewards redundancy. A class with broad sanity support, reliable anomaly handling, and useful inventory access can outperform a specialist even if the specialist has a stronger peak encounter advantage. Surgeon is the most defensible starting recommendation because it is reported as free to play and consistently appears near the top of the available lists. Head Nurse is the premium alternative if its current kit covers your weaknesses.
Coordinated teams
Teams should assign jobs before comparing letter grades. One player can focus on anomaly encounters, another on sanity or recovery, and another on supplies and access. Secret Agent may be more valuable in a group that lacks combat coverage, while Psychologist, Doctor, Security, or Paramedic may be better when the group fails through attrition. Avoid buying duplicate roles simply because they share an S or A label.
Common tier-list mistakes
- Calling a guide ranking official. The available lists are editorial judgments, not developer rankings.
- Blending conflicting tiers. Head Nurse, Secret Agent, and Secretary have materially different placements across the two detailed lists; preserve that uncertainty.
- Assuming premium means best. Robux status indicates a currency category, not a verified performance guarantee.
- Using an incomplete roster. Firefighter and later-reported Scientist and Barista may not appear in older references.
- Quoting unverified prices or statistics. No official current price table or independently measured class performance was established.
- Ignoring playstyle. A class optimized for combat can be a poor choice if your actual losses come from sanity or supply management.
What to verify before publishing your own tier list or spending currency
- The complete class roster visible in the current game client.
- The displayed unlock currency and price for each class.
- Current descriptions, cooldowns, restrictions, and inventory effects.
- Whether a recent patch changed anomaly behavior or class balance.
- Whether the class solves your solo or team’s actual failure point.
Until those checks are complete, the most accurate conclusion is conditional rather than absolute: Surgeon is the strongest low-commitment starting point in the reported rankings, Head Nurse is the leading premium all-rounder candidate, and Secret Agent is the premium specialist for combat-oriented anomaly play. Every other class should be ranked against the job you need it to perform, not against a tier letter detached from your shift.
