Some are, but not all. The Sims 4 includes pre-made characters as well as generated townies and Sims created or selected to fill particular roles. Which Sims you encounter can depend on the role and the save; the game’s official materials do not describe one universal process for every NPC.
What does “NPC” mean in The Sims 4?
Players often use “NPC” for Sims outside the active household. EA calls most of these characters “Neighbor Sims,” while excluding special NPCs such as the Grim Reaper from that term. The labels are not perfectly interchangeable, so “NPC” is best treated as a broad player-facing category rather than a precise description of how a Sim was made. EA’s Neighborhood Stories explanation sets out its definition.

Which Sims are pre-made, and which are generated?
The game has both designed, pre-made Sims and generated Sims. Community-maintained references describe many base-game townies as generated, and an EA Forums community guide explains how Sims may be selected or generated to fill gameplay roles. These sources are useful for understanding what players see, but the guide is not an official technical specification. The Sims Wiki’s townie reference discusses the distinction.

There is no supported basis for saying that every role follows the same selection order, or that the game always reuses any available Sim. The EA Forums community guide to NPC roles offers practical detail, but EA has not published a complete role-by-role specification in the sources available here.

Are townies the same in every save?
Do not assume that every townie or role-fill Sim will be identical across saves. The mix of pre-made and generated Sims means that the Sims you meet can depend on the save and on which roles need to be filled. The consulted sources do not establish a complete, universal account of what stays the same between saves, so they cannot support a guarantee that townies will always match—or always differ.
Does Neighborhood Stories generate NPCs?
No: Neighborhood Stories is described as a system for life changes among Neighbor Sims, not as the system that initially creates their identities or appearances. EA’s examples include career changes, having a baby with a partner, and forming or deepening friendly relationships. Its statement that “Neighbor Sims ultimately decide to initiate life changes themselves” refers to those changes, not to how NPCs are generated. EA’s explanation of Neighborhood Stories also says players can control its effects through settings, including rules for individual households.

Can you use Gallery households to control which Sims appear?
The Gallery lets players browse, save, and download community-created households, so it can help you add custom Sims to a save. However, the official Gallery information does not say that every NPC job or role will use a household downloaded from the Gallery. Treat it as a way to add player-created households, not a guaranteed way to replace every generated role-fill Sim. EA’s The Sims 4 FAQ describes the Gallery and its downloadable households.

EA’s consulted materials do not publish the NPC-generation algorithm, how often new Sims are generated, or a complete selection order for every role. Practical guides and community references can describe observed behavior, but they do not establish exact frequencies or a rule that applies to all NPCs. The reliable answer is therefore limited: The Sims 4 has pre-made Sims and generated or role-fill Sims, and not every NPC is random.
