Spoilers for Astarion’s story begin with his early companion scenes and continue through Act Three. Astarion is a recruitable high elf rogue and Origin companion in Baldur’s Gate 3. He is also a vampire spawn whose story follows his struggle with Cazador Szarr, his fear of losing control, and his pursuit of safety and power. Your choices shape his approval, romance, and the outcome of his companion quest, The Pale Elf.
Who is Astarion?
Astarion is a high elf rogue and one of the game’s Origin companions. Origin companions have substantial personal stories and can also be chosen as the player character. Astarion’s companion arc centers on his history as a vampire spawn under Cazador Szarr and the tension between wanting freedom and seeking power as protection.
The story explores enslavement, fear, bodily autonomy, intimacy, revenge, and the possibility of change. Those themes matter in practical conversations as well as in the major decisions of his quest.
How do you recruit Astarion?
You can recruit Astarion in Act One, shortly after the prologue. Find him on the Ravaged Beach. He initially mistakes you for someone connected with the mind flayers; once the misunderstanding is resolved, you can invite him to join your party.

When does Astarion reveal he’s a vampire?
Astarion is a vampire spawn. The game can reveal his secret in a scene during a long rest, when he attempts to feed, or in later dialogue if that scene has not happened. After the secret is revealed, he gains the Vampire Bite ability as a companion.
The timing and route to the reveal can vary with playthrough events. It is not dependent on a single mandatory scene.
What does Astarion approve of?
Astarion tends to approve of wit, deception, displays of power, and choices that support him or respect his autonomy. He can disapprove of certain displays of weakness, charity that brings no apparent reward, or decisions that restrict his agency. These are broad tendencies rather than a guarantee that every choice in a category will produce the same response.

You do not need to play an evil character to build a relationship with him. Understanding his past, helping him, backing his independence, and helping people or animals he cares about can also matter. Approval shifts depend on story progression and prior decisions, so treat any approval checklist as guidance rather than a universal script.

Yes. Astarion can be romanced by player characters of any gender. The romance develops alongside his personal story; it is not simply a reward for accumulating approval. How you approach intimacy and whether you respect his choices affect the relationship.
The Act Two choice involving Araj Oblodra
In Act Two, Araj Oblodra may ask Astarion to drink her blood. He makes clear that he does not want to. Pressuring him can have serious narrative consequences: in a later conversation, he describes feeling that he can no longer use his body as a tool. If you are romancing him, pushing him into the encounter can damage the relationship, although subsequent dialogue can affect what happens next.

How do Astarion’s quest choices affect his ending?
Astarion’s companion quest is The Pale Elf. Its objectives and journal entries can change according to earlier story decisions. Choices near the end of the quest lead to sharply different outcomes for his pursuit of power and his future. Rather than treating one ending as inevitable, consider what the decision means for his autonomy and whether it advances or frustrates the kind of power he wants.
- Agency: Does the choice respect Astarion’s ability to decide what happens to him?
- Power: Does it help him pursue power as a way to feel safe, or challenge that pursuit?
- Relationship: Does the decision fit how you have handled trust and intimacy with him?
These questions can help clarify the stakes, but they do not replace the game’s specific consequences. The quest’s progression and scene triggers may depend on what has already happened in your playthrough.
