Blizzard’s Season of Discovery Phase 4 announcement gives broader context for the phase’s content, including dungeons and raid content; it does not establish whether this item’s drop table remains unchanged today.
The available item listing does not state a drop rate, guarantee, lockout, or loot-eligibility rule. Treat Avatar of Hakkar as the listed source, not as a guarantee that the medallion will drop on a particular kill.

These are three distinct trinkets with different listed roles. Check the full name before pursuing or equipping one:

Don’t confuse it with Atal’ai tokens
Atal’ai Blood Icon and Atal’ai Ritual Token are separate items, not alternate names for the medallion. Their item references say they drop from Temple of Atal’Hakkar bosses and can be exchanged with Zalgo the Explorer on Yojamba Isle for the gear listed on their respective pages: Atal’ai Blood Icon and Atal’ai Ritual Token.
