There are four Mystic Carrion jars in Baldur’s Gate 3: brain, liver, lungs and heart. The heart jar is the one to destroy if you want to stop Mystic Carrion from returning; the other three are optional targets that weaken him. The brain and liver are in the Ancient Lair, the lungs are in the Lower City Graveyard Mortuary cellar, and the heart is inside Thrumbo until you reveal the clue that makes him expel it.
Where to find the Mystic Carrion jars
Brain jar: Ancient Lair
Look for the brain jar on a table among the remains in the Ancient Lair. It is one of the optional jars: destroying it weakens Mystic Carrion, but is not required to prevent his return.
Liver jar: locked chest in the Ancient Lair
The liver jar is inside a locked Gilded Chest in the Ancient Lair. A community walkthrough lists a DC 20 Sleight of Hand check to pick the lock; the check can depend on your character or approach, so treat that number as a guide rather than a universal requirement. BG3 Wiki’s Mystic Carrion page also identifies the lair jars and their effects.
Lungs jar: Lower City Graveyard Mortuary
Go to the cellar of the Lower City Graveyard Mortuary. The lungs jar is on a table there.

Heart jar: inside Thrumbo
The heart is not initially waiting in a room like the other jars. In the Ancient Lair, read the book Funerary Jar Form and Function. It reveals that the heart jar is hidden inside Thrumbo. Return to him and tell him what you learned; he will expel the jar, allowing you to take it and destroy it.
Which jar matters for permanently defeating Mystic Carrion?
Prioritize the heart. Community guide references report that Mystic Carrion can resurrect while his heart remains intact. Destroying the brain, liver and lungs weakens him, but those jars are not required for a permanent defeat. If you want the relevant choice at a glance:

Destroy the heart jar safely
The heart jar explodes when destroyed. The BG3 Wiki lists the blast as 8d6 fire damage; use a ranged attack and keep your party away from it. The figure is attributed to the wiki’s current page as retrieved October 7, 2026, not to an official developer statement.

There is an alternate quest state: if Mystic Carrion kills Thrumbo and you take a long rest before destroying the heart, the heart jar is reported to appear in the basement of Philgrave’s Mansion. This is not the normal first location; ordinarily, discover the clue in the Ancient Lair and ask Thrumbo to expel the jar.
