They are shaped by your choices and can appear when you load an existing save. “For every character” has limits: Larian’s announcement does not mean every companion, NPC, or custom build receives a separate cinematic.
What did Patch 7 add?
Larian Studios announced 13 new cinematics for evil playthroughs in its Patch 7 release post on September 5, 2024. The studio described them as offering “varying conclusions to your most evil playthroughs.” The update’s July preview had outlined routes for avatar Origin characters, including the Dark Urge, as well as multiple paths for custom player characters.
The release announcement says the new cinematics include unique endings for each Origin Character. That is the basis for the “every character” framing: it refers to the Origin-character routes and applicable player-character paths, not a separate cinematic for every companion or NPC. The sources do not provide a complete scene-by-scene inventory of all 13.
How do the endings appear?
The cinematics are tied to player choices, so the ending you see depends on what you did during the run rather than simply selecting an ending from a menu. Larian also said the new content can appear when loading an existing save; starting a fresh playthrough is not a stated requirement.

Can you turn off the cinematics?
Patch 7 renamed the former “Show Cinematic Nudity” option to “Show Sexual and Violent Cinematics.” The setting includes the new evil-ending cinematics along with sexual content involving nudity and selected particularly violent scenes with gore. Larian says switching it does not alter the adventure, choices, or access to romance; it controls the depiction of that explicit or extreme content.

These are Patch 7’s historical release dates, not a statement that Patch 7 is the game’s current version. The PC announcement also described a roughly 11.6 GB download and 160 GB of free space needed for that release installation; those figures are historical PC installation details, not current requirements.

Patch 7 changed more than endings
The evil cinematics were one part of a broader update. Larian also highlighted Honour Mode changes, new Legendary Actions, split-screen improvements, official mod support, an in-game Mod Manager, the modding Toolkit, and other fixes. The Toolkit was described as a free, separate Steam application.
