Spoilers for Lae’zel’s story: it depends which order you mean. At Crèche Y’llek in Act I, walking away from Vlaakith’s command can make Lae’zel attack. At the Act III camp visit, refusing the order to kill Orpheus has no reported immediate consequence and leaves an alternative quest route open. These are separate decisions, so don’t assume the Act III outcome applies to the crèche.
Which Vlaakith order are you deciding about?
Vlaakith confronts the party in two distinct scenes relevant to this choice: first at Crèche Y’llek in Act I, then through a projection at camp in Act III. The Act I command concerns the being inside the Astral Prism, whom the party knows as the Dream Guardian. The later order is for Lae’zel to kill Orpheus. Their consequences differ.
Act I: What happens if you refuse at Crèche Y’llek?
Larian’s Patch 6 notes say that Lae’zel can attack if the party walks away without carrying out Vlaakith’s order. If keeping the party together in the moment is your priority, do not treat walking away as a consequence-free refusal. The available evidence establishes this particular risk, but not a complete list of every dialogue route or approval effect. Larian’s Patch 6 notes
Act III: What happens if you refuse to kill Orpheus?
The camp visit and Lae’zel’s response
According to the quest reference, Vlaakith’s projection can appear during a Long Rest after the party enters Rivington. She orders Lae’zel to kill Orpheus and promises to make her Chosen. Depending on prior events, Lae’zel may reject the order outright. If she has learned the truth from Voss but is still being turned back toward Vlaakith’s side, the reference describes a DC 30 Persuasion check or DC 30 Wisdom option to persuade her to obey. Refusal does not mean an immediate attack
The same reference says refusing this Act III order has no direct immediate consequence, although Vlaakith threatens that dragon-riding warriors will hunt Lae’zel. The threat is not described as an immediate combat encounter. The subquest can be resolved later, after the party gains access to the Astral Prism or after the Netherbrain fight.
What refusal means for the route
If Lae’zel rejects Vlaakith, her story can continue along The Prince of the Comet route. Obedience instead keeps her on the Vlaakith’s Will path in the quest’s framing, where she is tasked with killing Orpheus if he is freed. Refusing therefore preserves the alternative Orpheus route; it does not by itself settle every later decision about Orpheus’s fate.

How prior progress can change the scene
Lae’zel’s knowledge and commitment matter. What she learns from Voss can determine whether she rejects Vlaakith automatically or can be persuaded back toward obedience. Larian’s Patch 4 notes also say the Act III camp scene can occur before a conversation about meeting Orpheus. They describe dialogue behavior when a player leaves a Raphael conversation early, and clarify that Lae’zel can remain in the party if the player skips the crèche. For a Lae’zel story centered on questioning her ruler and seeking a different future for her people, refusal fits that arc. A GDC narrative review describes her story as a challenge to longstanding beliefs and a chance to claim a new path; that is a role-play interpretation, not evidence that one route is mechanically superior. Practical recommendation
If you are at the Act III camp scene and want Lae’zel to reject Vlaakith, refuse: the cited quest reference reports no immediate punishment, and the alternative route remains available. If you are at Crèche Y’llek, make the choice knowing that walking away can lead to Lae’zel attacking. There is no single best choice for every playthrough; the meaningful trade-off is immediate party stability at the crèche versus the allegiance and story arc you want for Lae’zel.
