Cosmetic packages are the natural place to start if you want to personalize the game without seeking a new gameplay challenge. Their multiplayer requirements still vary, so verify their listed scope rather than assuming a visual change is automatically local-only.
New gameplay
Packages adding items, levels, monsters, tools, upgrades, or weapons can change the substance and balance of a run. For these, coordinate with your group and follow the package author’s installation and multiplayer directions.

For gameplay mods, the installation guide recommends coordinating with the host or group and using matching game builds and mod versions. The package’s own multiplayer requirements take precedence. Semiwork has expressed general support for modding; PC Gamer reported the development team encouraging mod makers to optimize server traffic to avoid excessive server costs. That is not an endorsement of any particular package.
How to install R.E.P.O. mods
The installation guide recommends using a mod manager profile rather than treating manual downloads as the default. It describes Thunderstore Mod Manager and compatible alternatives including Gale and r2modman.
- Create a R.E.P.O. profile in your chosen mod manager.
- Install BepInExPack first. It is listed among the prominent R.E.P.O. packages and is the guide’s recommended first installation.
- Add the mods you want to the profile, checking their dependencies and package-specific instructions.
- Launch the modded game through the manager so it uses the profile you configured.
- For co-op, coordinate versions and scope with your group according to each package’s instructions.
The guide presents this as a workflow, not a claim that only one manager works. A manager profile can make selecting packages and dependencies more straightforward; if you choose manual installation, follow the mod’s own instructions rather than assuming the same steps apply to every package.

Game updates can temporarily break mods. Before installing or troubleshooting, check the package’s update and compatibility information. If a mod is outdated after a game update, disable it until its author updates it; for a co-op session, also check that players are using the game builds and mod versions required by the package.
