Treat illness quickly because vomiting and defecation lower hygiene, isolate dissent before it spreads, and let exhausted followers sleep before sending them on missions.

What each status means before you choose a cure
Illness, dissent, and exhaustion are not interchangeable debuffs. Illness is a health and sanitation problem, dissent is a loyalty crisis, and exhaustion is a fatigue state caused by overwork or strenuous activities. The fastest solution depends on identifying the icon and checking the follower’s current thought or status rather than assigning a random chore.

- Illness: the follower cannot work, may vomit or defecate around the settlement, and can eventually die if left untreated.
- Dissent: the follower stops working, spreads falsehoods that can influence other followers, and may leave with a substantial share of the cult’s coins.
- Exhaustion: the follower needs sleep. It reduces useful labor and is particularly harmful when the follower is being prepared for a missionary trip.
Open the follower list or select the affected follower directly, then use the status-specific interaction described below. Do not try to solve exhaustion with food, or dissent with a medical building: those actions do not address the underlying state.
How to cure a sick follower
Option 1: Order Bed Rest
Bed Rest is the no-cost emergency treatment. Select the sick follower, choose the Bed Rest command, and leave that follower out of the work rotation until the status clears. This costs no Camellias, but the follower is unavailable for production while recovering. It is the sensible choice when the sickness is isolated, your Camellia stock is low, or the settlement can absorb a temporary loss of labor.

Keep the resting follower away from additional chores. A sick follower cannot work effectively and can make the settlement dirtier, so treating the status without cleaning the surrounding area can create more sickness after the recovery ends.
Option 2: Use the Healing Bay
The Healing Bay provides an immediate cure for a sick or injured follower. Select the affected follower, interact with the Healing Bay, and pay the listed Camellia cost. The building has no cooldown between treatments, so several sick followers can be treated one after another if you have enough flowers.
The building description is concise: “A station for you to heal sick Followers.” It can be built once and then upgraded to Healing Bay II. The documented construction and treatment requirements are:

These are in-game costs documented for the Healing Bay and its upgrade. The upgrade lowers the Camellias required per treatment; it does not turn treatment into a free service. Build the first bay when bed rest would repeatedly interrupt important production, then consider Healing Bay II when frequent treatments make the lower per-use cost worthwhile.

Why untreated illness gets worse
Sick followers can vomit and soil the settlement through defecation. Those messes reduce hygiene and can expose healthy followers to the same sanitation problems that caused the original sickness. Clean vomit and poop as soon as you see them, and check food preparation areas after an illness event. A follower who remains untreated may eventually die, so leaving the icon on the follower list is not a harmless way to save resources.
Common illness triggers and prevention
Low sanitation, certain events, and some food effects are the main practical causes. Keep toilets usable, remove contamination promptly, and avoid allowing food production and waste to accumulate unchecked. Maintain a Camellia reserve before events that can affect many followers at once.
The Brainwashing Ritual deserves special caution. It can be followed by widespread illness, with the risk especially pronounced when the Belief in Prohibition doctrine is active. Before starting that ritual, make sure you have enough Camellias for several Healing Bay treatments or enough beds and spare labor to use Bed Rest.

Automating medical trips with a Medic Station
Where the relevant content is available, a Medic Station can assign a follower to escort sick and injured followers to the Healing Bay automatically. This reduces the need to notice every new medical icon manually. The systems reference lists exceptions for Cursed Followers and Rotten Followers under the Heal the Decay doctrine; those cases are not handled by the normal Medic Station behavior, so inspect the follower’s special status rather than assuming an automatic escort will work.

How to stop a follower from dissenting
Recognize a loyalty crisis early
Dissent usually follows a severe drop in faith. The dissenter will not work, can spread falsehoods to nearby followers, and may eventually leave with a substantial amount of the cult’s coins. Look for the dissent indicator in the follower list or follow the thought bubbles that identify the unhappy follower. Treating the first dissenter quickly is safer than waiting to see whether the mood improves on its own.
Use Prison and Re-educate
- Find the dissenter through the follower list or by selecting the follower displaying dissent.
- Build or select a Prison and place the dissenter inside it. Imprisonment prevents the follower from working against the cult or recruiting additional dissent.
- Enter the Prison interaction and choose Re-educate.
- Repeat Re-educate once per day until the dissent meter clears. A severe case can require several in-game days.
Re-education is the dependable route because it directly reduces the dissent state. Do not release the follower simply because one interaction has been completed; check the meter and continue the daily cycle until the state is gone.
Repair the reason faith collapsed
Prison controls the immediate threat, but the cult still needs to recover the faith conditions that caused the crisis. Feed hungry followers, clean the grounds, resolve active sickness, and use sermons, blessings, or suitable rituals to rebuild morale. If several followers are hungry or surrounded by waste, re-educating one dissenter without correcting those conditions can produce another dissent case soon afterward.

The Disciple trait allows a follower to re-educate dissenters and use Disciple Shrines. Once unlocked, it provides a useful way to distribute loyalty-management work instead of requiring the leader to perform every interaction personally. It does not remove the need to address hunger, hygiene, sickness, or low faith; it changes who can carry out the re-education task.

Why Minced Follower Meat is a gamble
Minced Follower Meat is described as an emergency shortcut that may stop dissent, but it carries an illness risk. Choosing it trades a possible immediate loyalty improvement for the possibility of creating a medical problem that removes the follower from work and can contaminate the settlement. Use Prison and repeated Re-educate when you need a predictable result. Consider the food option only as a deliberate resource trade-off when you accept the medical consequences.
How to get rid of the exhaustion icon
Send the follower to sleep
Exhaustion normally clears through sleep. Select the exhausted follower and send them to bed rather than assigning another chore. Bed Rest clears the condition faster than simply leaving the follower in the general work schedule, so use it when you need the follower ready for an upcoming task.
Do not force an exhausted follower to work through the night unless the short-term production is worth the later recovery time. Overnight work can create exhaustion, and the resulting loss of productivity can exceed the labor gained from one additional shift.

Night work is the most visible cause, but exhaustion can also follow a missionary’s return, being used as a demon during crusades, breeding, and other strenuous systems. Check the follower after any of these activities before assigning another demanding job. A follower who looks available in the work list may still need a full sleep cycle after returning from one of these actions.

Protect mission success
Exhaustion lowers mission success odds in the cited systems reference. Schedule a missionary assignment after the follower has slept, and avoid waking a sleeping follower merely to gain a small amount of production. If a mission is urgent, compare the risk of a reduced success chance with the value of the immediate labor; for normal missions, allowing the sleep cycle is the safer decision.

The trade-offs are different in each row. Sickness is primarily speed versus Camellias, dissent is reliable control versus a risky shortcut, and exhaustion is short-term labor versus recovery and mission readiness.
Settlement routines that prevent all three states
Keep sanitation ahead of the problem
- Inspect toilets and remove poop before it spreads through high-traffic areas.
- Clean vomit immediately when a sick follower produces it.
- Watch food production and storage so spoiled or problematic food effects do not add to the illness load.
- Leave enough spare labor to clean instead of assigning every follower to a production station.
Maintain medical reserves
- Keep Camellias in reserve rather than spending every flower as soon as it is harvested.
- Plan for several treatments before a Brainwashing Ritual or another event that can trigger mass sickness.
- Build the Healing Bay before the settlement becomes so large that multiple Bed Rest orders disrupt essential production.
- Upgrade to Healing Bay II when repeated treatments make the lower 10-Camellia price more valuable than saving the upgrade materials.
Protect faith and morale
- Feed followers consistently so hunger does not become a faith crisis.
- Use sermons, blessings, and appropriate rituals to restore morale when faith falls.
- Resolve illness and clean the grounds before low hygiene creates another source of discontent.
- Check the follower list and thought bubbles regularly instead of waiting for dissent to spread.
Plan work and missions around sleep
- Let exhausted followers sleep rather than adding another chore to the schedule.
- After missionary returns, demon use in crusades, breeding, or other strenuous activities, allow recovery before a new assignment.
- Do not wake a sleeping missionary unless the production gain justifies a lower mission success chance later.
- Use Prison and daily Re-educate consistently when dissent appears; delaying the first interaction gives the problem more time to influence others.
Troubleshooting when the status does not clear
The sick follower is still ill after Bed Rest
Confirm that Bed Rest was actually assigned and that the follower has access to a bed. If you need an immediate result, use the Healing Bay and verify that the Camellia payment was accepted. Then clean any vomit or defecation left in the settlement; the medical status and the sanitation problem are separate tasks.

The dissenter leaves Prison but still dissents
One Re-educate interaction is not always enough. Re-enter the Prison interaction each day and continue until the dissent meter clears. While waiting, restore food, hygiene, sickness, and faith conditions so the follower is not returned to the same environment that caused the crisis.
The exhaustion icon returns
Look for a new strenuous activity or a forced overnight shift. Missionary returns, demon use during crusades, breeding, and other demanding systems can apply exhaustion again after a previous sleep. Give the follower another uninterrupted rest period and avoid scheduling a mission before recovery is complete.
