To upgrade a standard weapon in Dark Souls, reinforce it with a blacksmith or a Weapon Smithbox, then take it to Andre of Astora for the two key ascensions: +5 to +6 and +10 to +11. Reinforcement and ascension are different operations
Reinforcement raises an item along its current path, such as taking a Short Sword from +3 to +4. It costs souls and the required Titanite. A blacksmith can reinforce weapons, or you can do so at a bonfire after acquiring a Weapon Smithbox.

Ascension is a gated transition, such as moving a standard weapon from +5 to +6 or changing it to a different upgrade path. Ascension requires the appropriate blacksmith and, at key standard-path breaks, an ember. A Weapon Smithbox does not let you perform ascensions.
How to take a standard weapon from +0 to +15
The material quantities below are for the standard weapon route described by the community-maintained Dark Souls Wiki. Costs are listed by stage so the Titanite Chunk needed for the +10 to +11 ascension is not confused with the chunks used for later reinforcement. Check the weapon’s in-game upgrade display before gathering materials.

These are ore counts; reinforcement also costs souls. The cited upgrade references differ in how they summarize the Chunk total around +10 to +11, so the table separates that ascension cost from the listed +11 to +14 reinforcement costs rather than giving one combined total.
Where to go when an upgrade is unavailable
Andre of Astora
Andre is in the old church building in Undead Parish. The community-maintained Dark Souls Wiki says he can reinforce weapons and, after receiving the Large Ember, perform the standard +5 to +6 ascension. The cited standard route also assigns the +10 to +11 transition to Andre after he receives the Very Large Ember.

Bonfire reinforcement
Once you have bought a Weapon Smithbox, you can reinforce eligible items at a bonfire. If the upgrade you need is an ascension rather than a normal reinforcement, take the weapon to the appropriate blacksmith instead.
Why a weapon may not reinforce
- You are at an ascension break. Standard-path upgrades from +5 to +6 and +10 to +11 require the corresponding ember and Andre; a smithbox cannot substitute for him.
- You are missing the listed ore or souls. Reinforcement requires both, and the required Titanite changes as the weapon advances.
- You are using the bonfire without a Weapon Smithbox. The box must first be purchased; it enables reinforcement, not ascension.
- The weapon follows another path or has different eligibility. The standard +15 instructions do not establish the requirements for magic, divine, fire, lightning, chaos, crystal, boss, dragon, or unique upgrades. Check the in-game options and a reference specific to that weapon and path.
What the standard route does—and does not—tell you about damage
Standard upgrades can increase base damage and may affect Strength or Dexterity scaling, but those effects depend on the weapon and path. Do not assume every upgrade branch preserves the same damage type or scaling. When comparing paths, check weapon eligibility, damage types and stat scaling, required smith and ember, material rarity, maximum level, and repairability.
