Yes. Counters stay on a creature when it transforms in Magic: The Gathering. Transforming changes which face of the same permanent is showing; it does not remove the permanent from the battlefield or create a new one.
What happens to counters when a creature transforms?
The permanent keeps the counters it already has. A +1/+1 counter, lore counter, or any other counter is not removed just because the permanent changes faces. Wizards of the Coast states this directly in its Innistrad: Crimson Vow release notes.
There is no separate counter-moving step: the counters remain on that permanent through the transformation. The face change alone does not add, remove, or copy counters.
Why do counters remain?
Transforming changes the face showing on a double-faced permanent; it is not a leave-the-battlefield-and-return event. Wizards’ FINAL FANTASY mechanics article explains that a transforming double-faced permanent remains the same permanent, so its Auras, Equipment, and counters stay with it.

For the same reason, continuous effects from resolved spells or abilities and spells or abilities already targeting the permanent on the stack continue through the transformation, subject to the ordinary rules—for example, an Aura still has to be legally attached.
Does the rule also apply to tokens and to “convert”?
Transforming double-faced tokens
Eligible transforming double-faced tokens keep their counters when they transform, just as eligible double-faced cards do. This includes Incubator tokens discussed in Wizards’ March of the Machine release notes. That does not mean every token can transform: an instruction to transform an object that is not represented by an eligible double-faced card or token may be ignored. Transformers cards that convert
Transformers cards use the related keyword convert. Wizards says converting functions similarly to transforming, and confirms that counters remain after a permanent converts in The Brothers’ War release notes. Cards that trigger when a permanent transforms can also trigger when it converts, as explained in Wizards’ The Brothers’ War mechanics article.

- If an eligible double-faced creature transforms, it keeps its counters.
- If an eligible transforming double-faced token transforms, it keeps its counters too.
- If a Transformers permanent converts, its counters remain.
- If an object is not eligible to transform, the instruction may be ignored; the instruction itself does not make every token or permanent transformable.
